I have had only wonderful experiences with Form Assembly. Great GREAT Customer service.
However, if you later want to go back and edit the questions present, or add one, the answers get completely screwed up.
This is great tool, especially when you don't know coding and you need to build a form and integrate with salesforce. We have built many forms so far and it works wonderful.
To be specific, there should be a way to customize the error message when the form is filled incorrectly by the user. Right now, the error message the form generates is not user friendly.
I love that I can create surveys on Form Assembly that make it very easy for the people completing the surveys to fill out. I get all of the information that I need in a clean and organized way.
After setting up the connection with Salesforce, it was a bit difficult for my client to go in and edit form fields as necessary - they ended up breaking some things.
Overall, Form Builder is a great time-saver. It is easy to use and does exactly what it is supposed to do.
Because, if there is and empty row on the sheet where the information were sent, is was creating an empty question. So I had to fix it manually after.

I found the facility to import from PDF's most useful and saved me time. I also used the forms as exit slips and check-ins for understanding.
Lack of customization for user interface & integrating formula in Google sheet.
I like the fact that you can easily create forms invented can save you plenty of time creating one from scratch.
It's more of a Goodie API thing, but I don't like that I can't use the links that haver my images. A few work but anything over 10 (probably, haven't tested) image links gives an error.
