Surveys done via Fieldboom first of all look amazing - very contemporary flavored and overall aesthetic to the eye. I love that there are many features to compile the data after a survey is completed.
This is a Scam don't use their service. They also went out of business and stopped working in November without even emailing us, costing us millions in lost advertising money (form didn't work).

I think the tool is good if you have fairly little amount of users and you will always send the invites out from another solution. In this case the tool is good value for money.
