We also needed a place were we could have an online application for new volunteers. They fill out the form, we get an email, and the info is attached either pdf or plain text on a second email. Pros Easy Setup with prompts and drop and paste info in to the fields. Its options to build your form from scratch is easy even for a novice. They even keep a record of all submissions for later review even if you delete the email notification. Even though, we use the free version, the prices are reasonable and comes with great service. Cons Using it too much can lead to a paid version, The product just works! We are trying to digitize things as much as possible as it is sometimes difficult to get signatures back from parents on things like special excursion permission forms. Jotform is by far the best we have used. The ease of creating the paperwork is great! Patients love that they can complete their paperwork and sign it conveniently. Cons We have not had any negatives yet, and have been using Jotform sign for a few years. Switched from [Adobe Express](https://www.capterra.com/p/174232/adobe-express/) We couldn't customize our forms easily. Our originals had to be scanned into the program and it was not easy to change what we needed to. Patients complained about only being able to use certain browsers to access and sign the paperwork. It's so easy to use, you can customize and start from scratch, or you can also upload your own template. Super user friendly. Pros I love how easy it is to create custom forms/docs for clients! Since it's digital, my clients can fill out forms and sign contracts on any device. Also, I love how I am provided a copy of contracts which makes it so easy to go back to reference. Cons Unless you use a paid version, all of your forms will have the Jotform logo. Also, until you really understand the website and/or app, navigating can get confusing. For a while I didn't know how to obtain the "sign docs" because I was only able to view my forms.
