Great connection to our post acute providers. As a leader, the reports help drive initiatives with our post acute partners.

Pros

Communication with post acute vendors directly in the referral. Utilizing care port as a source of truth for referrals.

Cons

it’s not integrated yet into epic. We are going to be working on getting care transitions implemented.
