Overall, I found the quality to be top notch. Navigating the tool within Salesforce was not as user friendly as the standalone version. The detail, the customizability, and the broad range of integration is what makes this tool so good.

Pros

While this tool has a bit of a learning curve compared to other form tools, it is very high quality. The form creation aspect of it is powerful. It offers a very wide range of options and customization. While I did not use it to for any payment integration, they worked with all the major players in that market. With its native Salesforce integration, this tool is perfect for those needing to embed in the Salesforce house.

Cons

The tool was a bit overwhelming. Which is ultimately what makes this product work so well. It has a lot of moving parts.

### "DO NOT USE FORMSTACK"

Where do I even begin... Formstack is HORRIBLE across the board. To start... Absolutely criminal pricing. Second... Next to impossible to cancel...HUGE waste of time, there's no cancel button. I've been trying to cancel for 2 months... and in the meantime they have been billing me for those two months. And also they have just let me know that apparently I am on a yearly contract?? And I can't cancel for 8 months? After I spend another $4k+ with them?? I am so beyond disappointed. I have been a customer for 5+ years. It's an insanely expensive, subpar service. I am so beyond my limit with Formstack. On top of all of this, I have reached out when our form logic was just NOT working (REALLY bad look for us for our clients) and they gaslit me and told me it was working after it had been fixed. SO why when I did absolutely nothing did my form not work and now it all of sudden works??? Hm... Go with another form provider. My business needs HIPAA compliant forms and there are way better alternatives out there for a FRACTION of the price. Like less than what I am paying PER MONTH at Formstack for an entire year kind of fraction of the price. I would give 0 stars if I could. I've been getting ripped off for years and am mad and feel taken advantage of and now they are making it my problem. I could have saved $10k if i switched to another provider earlier. I wouldn't be so mad because I do take responsibility for my part in this... but it's also taken me HOURS to attempt to leave the service (they still haven't cancelled my account and each month that goes on they keep charging me!!!!), another 20 hours or so to rebuild my forms on another platform. I am beyond unhappy with them and FULLY recommend you go with a different form company.

Overall, formstack was a great choice for our organization. We've setup and deployed dozens of forms for both public facing use and internal use.

Pros

Formstack's form builder is easy to use and has a lot of integration options. Our form building users range from not-so-tech savvy to power users and both are able to create forms with ease.

Cons

Lack of variety with field types. While formstack isn't a "survey" tool, it's a form tool, I think they could implement a lot of field types that survey tools use and make it easier to use a single platform for all form/data collection needs.

Reason for choosing Formstack Forms

While FormAssembly came highly recommended for it's well built Salesforce integration, we found for our needs Formstack was nearly as capable and the pricing per user was much better. With Formstack we get 10 user accounts where I believe a similar price range with Formassembly was for a single user.

### "Formstack didn't meet our needs during development and now we're stuck with the bill"

At first, our experience with Formstack was promising. However, it clearly became evident that formstack for Salesforce was ill-equipped to handle extensive forms with little formatting needs without passing data to the object.

Pros

We like that the product was a native salesforce integration and that it was capable of attaching the response PDF to the account object. Unfortunately, Formstack failed when it came to our formatting needs.

Cons

The BioSpine Institute needed a short term patient paperwork solution to attached the response PDF to the Account Object. Unfortunately, we ran into several PDF formatting and rules issues. Our forms required hundreds of rules to hide and show fields based on the patient's input. The rules engine was not user friendly and cumbersome, forcing the user to search for existing rules by clicking through pages. We spent weeks trying to make it work. Our PDFs were 26 pages long when they should have been closer to 8. It was so long because hiding certain pages was impossible. Formstack Forms produced an unacceptable product as it would slow and hinder Clinic operations. Formstack's solution? Formstack suggested using Documents, a feature we had not purchased, to format the PDF the exact way we wanted it. Unfortunately, this would require us to create hundreds of fields in Salesforce and pass the data from the form into Salesforce. We specifically discussed during the sales process that we wanted very little if any patient data in Salesforce. If this were known at the time, we would have looked for another solution. After multiple meetings and communications, we expressed our desire to cancel the contract due to Formstack's inability to meet our needs as promised. However, Formstack denied our request and is currently billing us every month for a product we can't use. If you're looking for a form solution, don't use Formstack for Salesforce.

We need a native form solution that could attache the response PDF to the Account Object. This works, but you have little control over formatting without also purchasing the Documents product. But in order to use the documents product, you must build the fields on the account object to store the data. This was a no go for us and discussed during the sales meeting.

### "A great form builder for any online application."

Great, couldn't be better. You can produce forms very easily with a drag and drop interface. Styling the forms within Formstack is ok; perhaps not as many freedoms as other products but it did everything I needed it to do.

Pros

A really simple entry point into building online forms for any website. Simple to use as a standalone product i.e. producing a form for an organisation and hosting direct with Formstack; or embedding so it's on your own URL.

Cons

There isn't really anything I didn't like. I simply started working more with Wordpress and found it easier to deploy Gravity Forms given it's native within Wordpress. I believe that Formstack now also offers this.

Overall Formststack Fotm is quite imquitibe. It interoperates well with my OS and summarily, it's amazing that onboarding process is smooth and the application combines well with others.

Pros

I like the way Form Stack works. It's quite easy to fill out and navigate giving on qiestion at a time with corresponding progress flow display. It's quite flexible and easily personalosed.

Cons

Formstack Form progressive filling could sometimes be challemging especially for mew users. Going back the window coilf also ne difficult.

I've been using this tool for a while now and it's by far the most "high-end" form tool I have used. I did hear the cost is higher than others, but I can understand the reason for that.

Pros

What I like the most about Formstack Forms is that it has HIPAA compliance, which is extremely important as we are a healthcare company.

Cons

I don't really have any complaints, except maybe it's not that easy to learn as it's not as simple as other form tools, however, it's worth learning.

Cons

The setup is always a hassle. There is always some minor nuisance that breaks the form. You are lucky to have more than one communication from customer support per business day. So in order to get anything corrected, it takes almost an entire week.

### "Mislead by Staff in regards to Infusionsoft Integration, Won't Provied Refund From Their Mistake"

Everything up until discovering that the product was not as promised was good. The people I dealt with were kind and pleasant to work with. It was clear that they were under the same impression I was, they communicated this to me via email, apologized for the mess up and tried to make it better. However, the only thing that would make it better is a refund as I was still paying for all the other systems that this one was intended to replace but without the integration there was no reason why I would change and pay more for what I already had set up and in use. The whole purpose of changing was to consolidate and send the data to my CRM without using an integration tool so that we can create a HIPAA compliant integration and start sending patient data between the forms and the CRM which was not a possibility all along, or at least not at the price they quoted me but they never fixed the issue which was a major disappointment given the otherwise positive experience with their staff.

Pros

I initially liked working with the people, for the most part, I think the ones I delt with are more or less at the mercy of their employer and it appeared as though they were mislead on the capabilities so I don't hold them responsible.

Cons

I only discovered them by researching systems that integrated with Infusionsoft. After several discussions and the sales person researching the uses I needed and whether or not Formstack could provide those solutions, it was determined that it does offer the integration with infusionsoft and that we could build a form that we can use for checking clients in for appointments, collecting information through intake documents, onboarding new staff and clients. We discussed at length using the forms to collect patient data that could sync with our CRM, Infusionsoft and this was all confirmed. The price was quoted to me that would include Formstack, Docs, and Sign. It was more than I was paying for an electronic document system, an electronic check in system, an integration tool, and an onboarding / training tool however the convenience of consolidating all of these into one platform was the reason I purchased it. After a full month of trying to set it up with my rep, talking to the strategic accounts manager, more testing with my set up rep, it was finally determined that integration with infusionsoft was an additional custom coding project and would cost hundreds more a month. This was not what was communicated, I was invited to meetings with the developers to sell me an integration, I asked for a refund and and was happy to talk once they had the integration they advertised. They agreed but never gave me the refund. Their mistake cost me about $2,000 for an unusable product.

Reason for choosing Formstack Forms

I intended to replace IntakeQ, theReceptionist, Tranual, and Zapier for Formstack and integrate all of these into Infusionsoft without needing Zapier and also being able to integrate the forms that would allow patient data integrated with Infusionsoft. Something we are unable to do now with Zapier. So, I did not end up switching, overall I like those other programs but was looking to consolidate and streamline and since it wasn't possible I stuck with the other systems.

We use this for new customer registration and have it attached to a payment portal. It's great because the customer can check box all the agreements and use one signature for everything. Families can fill out all their information and Pay in less than 5 minutes. HUGE time saver and we get more business, because people don't have to ever stop and come back to it or flip through pages. If you are a martial arts school and are trying to figure out how to do new student intake and payment without using the really expensive companies like Mind Body or Zen Planner. I highly recommend using along side a merchant system like MXmerchant or a couple of the others Formstack has listed on their website and is compatible with. Pros

As soon as I started the trial it was easy to navigate through, and I could pretty quickly figure out what I was looking for. Everything is organized intuitively for a new user. That is probably what made me happiest about it. It also has great options for keeping data protected. That was something we struggles with a lot looking at other form generators. I can also generate a ton of different forms at no extra cost. I even use this if parents are signing for our annual potluck.

Cons

I don't get a lot of control manipulating the text for each field or heading. My biggest struggle is making the forms easy fora customers eyes to follow the page and be easy to fill out. When all the text is stuck the same size, the question and list of checkbox answers, it is kind of overwhelming, and customers in a hurry get lost and have to go back and re-read.

Deceptive B2B sales practice after we reached out to their sales team. Should have stuck to the $50 dollar per month product but they forced us to pay over $1,000 per month with a 1 year contract because apparently we require a BAA due to being a healthcare organization, even if we are in Canada where US law does not apply. Service was also not set up correctly and could not go live as intended. When we requested to cancel within a couple of days, never having gone live, they sent the $16k to collections. This company put a small business under a lot of distress. Sending threatening letters and court action, and has now in large part help push a business breaking even to insolvency and 4 people have lost their job now. Thank you!

Making the move from paper-based forms and needing to collect increasingly complex data, Formstack was able to meet and then exceed our expectations. The data export features were just what we needed at that time.

Pros

Formstack allows users to customize forms to meet their needs. It simplifies both the form design and the collections process. For form design, it offers an intuitive interface that is easy to get started with and includes many ways to control the behaviour of each section on the form. What we liked most about FormStack was the quality control features they had built in to make that forms were being used correctly. Submitted forms could either be approved or rejected before being added to the data pool.

Cons

We didn't have any paper-based issues with Formstack. Formatting the look of the forms themselves was a bit of a pain sometimes with limited options available in the interface.

I love how easy Formstack is to use. Building forms is a breeze , the support is great, and we love that it integrates seamlessly with Unbounce to give us quality, high converting lead pages. Formstack has helped us refine and define our lead capture process and following up with qualified prospects has become even easier. We use Formstack for all of our lead capture across our whole website. Since starting with Formstack over a year ago, we're seen an average lead conversion rate of around 30% -- and these aren't just any leads, these are warm, marketing qualified leads.

Cons

We're happy with Formstack's functionality, however we're not a big fan of their updated Dashboard in the platform. We used to see a screenshot of big picture stats and our average conversion rate. Now we see a listing with each of our forms, but we don't feel like we're getting the value we used to get through this update.

Pros

We used Formstack as a first step of transition from having no online registration or information collection to accepting registration and payments online. I liked that it was fairly simple to create forms for information collection, but that information then had to be exported and transferred over to whatever system we used to store it.

Cons

While the basic drag and drop elements of the form were simple, it was not as straight forward to link it to payment software or collect payments. There were several instances where a box wasn't checked or the formula was not set up correctly by myself, another staff member, or volunteer and because of this payment didn't go through and had to be re-collected. If you are a religious organization or non-profit, I would advise collecting information, registration, and payment through a database system or something that's more all encompassing rather than using

### "I heard you like forms in your forms"

Great experience. We still use it for complex logic based forms immensely. It allows for so many if statements to capture exactly what we want from the person filling out the form.

Pros

Ease of use. Drag and drop is something people just get. Training a staff member on how to build their own forms only takes a few minutes and they get the basics and are off to the races with their work quickly. When you empower your staff to do their job and more it means less work for you so you can focus on the things they can't do. LOGIC! Logic is amazing and so fluid and helpful.

Cons

Cost structure. We have lots of small forms and lots of users, but formstacks structure does not like that. If you build fewer forms and lots of responses it works great. Lots of forms with smaller reponses you hit the cost barriers quickly. Of course if you delete the data it works fine, but if you give formstack as a tool to your staff and you don't want to manage the amount of forms it becomes unwieldy quickly.

There are definitely parts of this system that I appreciate and find very useful, but I think there are a few areas it could improve to really perfect the experience.

Pros

Easy to use UI with pretty straightforward features and options for building out forms. Includes some helpful conditional options for specific steps on forms, which was key to a lot of the forms I've had to create.

Cons

The main complaint I have for this system is that the conditional settings (which I had to use a lot of) would erase whenever possible answers in the drop-down list for a question were adjusted. So even if I kept the name of a certain answer the same, but adjusted the positioning of it, the conditional I built around that answer would disappear. Just wish the system could have been a bit smarter with that.

Pros

Formstack is simple to set up and easy to use. You can teach yourself how to create detailed form because it has a very logical setup. The forms generally work exactly how you'd expect them to and editing is simple in case of changes.

Cons

There isn't a whole lot I don't like. It would be nice to have a 'free' tier like competitors have. I have worked with companies that wouldn't need more than a handful of submissions a month, and that puts FormStack out of their price range.

Pros

Very easy to use, you just have to drag and drop the fields you need in your form. If you are looking for an app that only makes regular forms this one is for you.

Cons

Not so powerful features, i have tried another free apps such as Google forms and it does almost the same the only difference is that those apps have the same functionalities for free!.

Very useful for collecting data for surveys, program registrations, and waivers.

Pros

It has many customizable question types and logic options for when to show a question or not. It is fairly easy to build your forms how you want and to view the submissions.

Cons

I wish there was a way to send forms/surveys through the program itself and not have to rely on your own mail server. Being able to send the survey multiple times only to those who have not completed the survey would be a huge plus. Increased logic options would be appreciated.

Enabled us to capture information and have it automatically put into salesforce.com. The tool provided some features that we weren't able to get with just Salesforce such as attachment management (the ability to require attachments at various places in your form).

Pros

I used the FastForms version of this software. This software is fantastic for basic form needs that require salesforce integration. The software provides an easy to use form builder with a styling tool. Advanced customizations are possible with css and javascript.

Although it is great that we can customize with css and javascript it would be better if some basic things were included out of the box without requiring javascript or CSS. I'd like to see page specific styling and character counters as out of the box features that don't require custom CSS and javascript. However their customer support is fantastic and they went above and beyond to ensure that the customizations I needed were provided.

### "Form got deactivated poor notice"

Pros

Good features and good functionality. Very good for integrating with google sheets and google drive to make systems more efficient.

Cons

I was moving from a different form generating software and so did the same that I was doing before. They have more sensitive information security which is good. I was given a warning and so I emailed back and after reading their info page re:information security about how I can comply with their policies. Replies were not succinct and while I was trying to figure out how to follow their rules they deactivated my form and disrupted the business operation. Sad face. And angry face at the same time I am sorry I live in Australia and it is 12am here when Formstack is operating. Sad and feel like they don't really care about me much.
