Formstack is HORRIBLE across the board. To start... Absolutely criminal pricing. Second... Next to impossible to cancel...HUGE waste of time, there's no cancel button. I've been trying to cancel for 2 months... and in the meantime they have been billing me for those two months. And also they have just let me know that apparently I am on a yearly contract?? And I can't cancel for 8 months? After I spend another $4k+ with them?? I am so beyond disappointed. I have been a customer for 5+ years. It's an insanely expensive, subpar service. I am so beyond my limit with Formstack. On top of all of this, I have reached out when our form logic was just NOT working (REALLY bad look for us for our clients) and they gaslit me and told me it was working after it had been fixed. SO why when I did absolutely nothing did my form not work and now it all of sudden works??? Hm... Go with another form provider. My business needs HIPAA compliant forms and there are way better alternatives out there for a FRACTION of the price. Like less than what I am paying PER MONTH at Formstack for an entire year kind of fraction of the price. I would give 0 stars if I could. I've been getting ripped off for years and am mad and feel taken advantage of and now they are making it my problem. I could have saved $10k if i switched to another provider earlier. I wouldn't be so mad because I do take responsibility for my part in this... but it's also taken me HOURS to attempt to leave the service (they still haven't cancelled my account and each month that goes on they keep charging me!!!!), another 20 hours or so to rebuild my forms on another platform. I am beyond unhappy with them and FULLY recommend you go with a different form company. In the beginning, it was not the easiest platform to navigate, and I ran into some frustration with locating forms and keeping up with submission alerts. However, once I spent more time looking around and learning the system, it became much easier to use. Overall, it is a solid app with a lot of value, though it can become a bit overwhelming when you are working with a high volume of forms. Pros What I liked most about Formstack Forms is that once I took the time to explore it, I could see that it is a very capable tool. It offers a lot of functionality and can be really useful for creating and managing forms. After getting more familiar with the platform, it became easier to understand how much it can do and how helpful it can be for organizing information. Cons What I liked least was that it did not feel very user-friendly at first. There was definitely a learning curve, especially when trying to find forms or stay alerted when new submissions came in. June 11, 2025 5.0 I've been using this tool for a while now and it's by far the most "high-end" form tool I have used. I did hear the cost is higher than others, but I can understand the reason for that. Pros What I like the most about Formstack Forms is that it has HIPAA compliance, which is extremely important as we are a healthcare company. Cons I don't really have any complaints, except maybe it's not that easy to learn as it's not as simple as other form tools, however, it's worth learning. May 8, 2019 3.0 I had a really stressful experience with Formstack. It has taken me over a month to create a form - that's not even finished. It glitches, it lags, there are technical difficulties even above their heads. Even though they in the end got on the phone with me, it took pushing and a lot of time that I did not have and then when it was created it still wasnt right. It is a never-ending battle and they have not offered to compensate $ wise which is unfortunate. I wish there was a bit more versatility to the type of questions you can add to a form, but overall it's pretty good! Cons Our chief complaint was how many workarounds we've had to use in order to get SalesForce integration to work for us. The Formstack for Salesforce product leaves MUCH to be desired (taking away all form customization available in regular formstack) and it has taken hours and multiple team members to be able to streamline our formstack to salesforce process. It's now pretty seamless but it shouldn't have taken that many resources for a simple form-to-opportunity functionality. This came from a frustration in the lack of functionality in list creation, updating, and note creation with Salesforce, none of which worked well at a high level within formstack. They used some legacy scripting which cost us alot to mitigate even though I had asked them to updated their legacy script calls. I always explain it to say it is like Google Docs on Steroids. User interface is great and the customer service has always been amazing. Rather than spending an hour googling some solution to a problem I am having with a form I can always contact them by email or phone and get a great response spoon. The integration with third party services are great as well as the custom notification emails sent to attendees of events based on how they fill out their form. I can't recommend them enough for people who are trying to do online registration with groups of people. Pros Ease of Use. Integrations of third party services, customer service Cons Really not much. Pros What I like most about using this software is the ability to gather information digitally. You can send out a link to your form and multiple people can make responses. One of my favorite features of this software is the ability to extract the information as an excel. It give you information all the way down the the exact time that the person submitted their response to the form. Cons What I like least about this software is the inability to find certain settings. It has numerous pre-configured fields that you can just drag and drop into your form. I really like the logic and how it is easy to understand and configure. The low price is hard to beat when needing a simple registration system. Cons There is no customized reporting. If you want to export any data from your form you have to export it all and then clean up the excel sheet to what you really want. I gave up when I realized I could not export my finished form to either Word or PDF. Pros I liked the ability to drag form elements into the form I was making. It was quick and easy to do so. I didn't need to know any code or complicated techniques. Cons It was missing simple features like the ability to export my finished form to a PDF or Word file. Both of these file formats are necessary to have if the end user does not have access to the internet but still want to fill my form out (most form software companies assume that all end users will have access to the internet and believe that a link to a finished form is sufficient). Additionally, I could not situate my form elements in a horizontal landscape orientation. As a result, the form I made with this software became one long, multi-page document. With a user filling this out on the internet, this is not an issue. If not on the internet, it is a very long document to go through. Review Source MW Mark W. We still use it for complex logic based forms immensely. It allows for so many if statements to capture exactly what we want from the person filling out the form. Pros Ease of use. Drag and drop is something people just get. Training a staff member on how to build their own forms only takes a few minutes and they get the basics and are off to the races with their work quickly. When you empower your staff to do their job and more it means less work for you so you can focus on the things they can't do. LOGIC! Logic is amazing and so fluid and helpful. Cons Cost structure. We have lots of small forms and lots of users, but formstacks structure does not like that. If you build fewer forms and lots of responses it works great. Lots of forms with smaller reponses you hit the cost barriers quickly. Of course if you delete the data it works fine, but if you give formstack as a tool to your staff and you don't want to manage the amount of forms it becomes unwieldy quickly. Review Source MH Mim H. Pros Easy to use UI with pretty straightforward features and options for building out forms. Includes some helpful conditional options for specific steps on forms, which was key to a lot of the forms I've had to create. Cons The main complaint I have for this system is that the conditional settings (which I had to use a lot of) would erase whenever possible answers in the drop-down list for a question were adjusted. So even if I kept the name of a certain answer the same, but adjusted the positioning of it, the conditional I built around that answer would disappear. Just wish the system could have been a bit smarter with that. Pros It has many customizable question types and logic options for when to show a question or not. It is fairly easy to build your forms how you want and to view the submissions. Cons I wish there was a way to send forms/surveys through the program itself and not have to rely on your own mail server. Being able to send the survey multiple times only to those who have not completed the survey would be a huge plus. Increased logic options would be appreciated.
