### "Clinicians and Small Practices Beware!"

I have been a TherapyNotes user and paying customer for claims submission and clearinghouse services for four years, and my recent experience with a claims issue has been extremely disappointing. Given that this is not the first clearinghouse issue I have encountered with this platform, it raises broader concerns about reliability and accountability. Due to a clearinghouse error affecting claims submitted through the TherapyNotes system, multiple claims were never received by the payer, resulting in over $2,000 in lost reimbursements for services I had already provided. By the time the issue was identified, the claims had fallen outside the payer’s timely filing window. What makes this particularly concerning is that these claims were marked as successfully sent within the system, creating the impression they had been properly transmitted when they had not. Because the issue was not identified in time, my ability to resolve it within the required timeframe was limited. When the issue was finally identified, the responsibility for correcting it was placed entirely on me as the provider. I was required to reconstruct claims, navigate appeals, and manually fax submissions. While TherapyNotes provided a document to support faxing, the burden of correcting the error and attempting to recover payment remained with me, despite having paid for a service intended to handle claims transmission. Communication around this issue was also inconsistent. We received an email in February 2026, but after speaking with another TherapyNotes user, I learned she did not receive any notification or the fax document, which raises concerns about how broadly and reliably this issue was communicated. There was also no initial offer to reimburse the claims that were never transmitted, leaving me paying for a service that was not performed. The ongoing runaround in trying to obtain reimbursement has been the most frustrating part of this process. Support staff were initially courteous, but communication became increasingly vague and, at times, evasive. I have been working with both a support supervisor (Heather) and a representative from the sales team (Lymari), as those departments are not connected. This required managing communication across multiple channels and resulted in confusion and, at times, conflicting responses. After multiple exchanges, I was issued an $11 credit, but I am still seeking reimbursement for the July 2025 subscription fee of $78.37. I was told that full monthly reimbursements are not typically provided for claims-related issues, but this is not a typical issue. The system failed to transmit claims that were marked as sent, the error was not identified in time, and I was required to manually complete the work the service was intended to perform. As of March 20, 2026, the request has been re-escalated with no resolution. At this point, the continued back-and-forth over a clear service failure, without acknowledgment of responsibility, is unreasonable. For clinicians running small practices, failures like this have real financial consequences. A system designed for billing and claims submission should not create this level of risk or shift the burden onto providers to repeatedly follow up and correct errors they did not cause.

Pros

It helps me keep my client panel organized by bringing documentation, scheduling, and client information into one system, making day-to-day practice management more manageable and efficient.

Cons

Billing has been unreliable. Claims were marked as sent but never transmitted due to a clearinghouse error, resulting in over $2,000 in lost reimbursement. The issue was not identified in time, and I was required to manually reconstruct and fax claims. Reimbursement efforts have involved prolonged follow-up despite paying for this service.

Absolutely fabulous and will be a customer forever! I appreciate all of their hard work and dedication to amazing customer service and to produce a top notch product.

Pros

I can't say enough how wonderful Therapy Notes is and the amazing workers who make Therapy Notes so effective and user friendly. This company is privately and as far as I know has no plans to become public which I really appreciate and find important. Additionally, they have pledged to not use user data to train AI models. During the Change Healthcare cyber attack Therapy Notes was absolutely fantastic and within a month were already pivoting to a new clearninghouse. Every step of the way they communicated to their users exactly what was happening and what the plans were.

Cons

Nothing! I have truly never had a bad experience with Therapy Notes. They are responsive, helpful and kind.

Wonderful! It has never given us any trouble and our team as a whole loves it! I have worked with Therapy Notes for 4 years now and it has always been consistent and easy to navigate!

Pros

Therapy Notes is so easy to use & user friendly! We have never had any trouble teaching a new employee how to schedule, they have always caught on within minutes.

Cons

Nothing. No complaints at all. The Therapy Notes platform is setup wonderfully. You can leave notes in a patient's chart, in their appointment alert box etc. There are also note sections for billing and patient info.

Pros

Therapy Notes is a great all around EHR/EMR/RCM software. It is focused on mental health/therapy and does a great job for the clinician being an easy to use and navigable site. The RCM piece of it is simple and does the job.

Cons

It would be nice to be able to have more information on claim rejections and how to resolve the issue within the software.

Very satisfied with Therapy Notes. I had to use another company for a short while and TN far outweighed them.

Pros

The customer service is consistently excellent. Also, it is a great value for the money. I recently began using the AI feature for note writing as it saves me a lot of time and works very well. It is now a joy to write notes!

Cons

I used the video capacity for multiple locations but have discontinued it. Also, sometimes the connectivity is not working, but that has been a minor issue.

It was not great, but it was a good experience. It worked for what was needed in the group practice, but I switched to a different option when I branched into my own private practice.

Pros

The ease of scheduling clients and being able to go from one note to the other with the click of a button was very helpful.

Cons

It did not have the customizable features - color coordination, various billing rates for different individuals, submitting paperwork online. And I don't think it had an online portal for scheduling/cancelling appointments.

I have been using TherapyNotes for a couple of years (under supervision and now on my own). Transitioning to my own practice and billing was made much easier with TherapyNotes. Highly recommend!!

Overall it’s been great using therapynotes. I docked one star just due to the frequency of glitches. It does seem to come and go in waves.

Pros

It is a simple EHR that is easy to navigate and keep notes organized. I like the newer feature of having assessments in the program to use with clients.

Cons

The most frustrating thing is that the program can be glitchy for the video conferencing, so I do have to have a back up alternative link for clients.

TherapyNotes was a solid software option for EHR, notetaking, appointments, and all of the other basic functions needed to run a practice.

Pros

TherapyNotes provides a quality platform on which to complete patient notes while maintaining HIPAA compliance. It is easy to use and its features feel tailored to a clinician's daily needs, allowing clinicians to spend more time on client support. It has decent value for the money, depending on your practice model and clinic's needs.

Cons

TherapyNotes didn't end up being the option for me as a provider in solo private practice, as I was able to identify other options which made more sense financially while keeping with reliable HIPAA compliance. That being said, TherapyNotes is a well known platform and very well may be the option for you, depending on your clinic's needs. It wasn't a bad software - just not the one that met my usecase.

I feel there are many things that could be improved. Reports and data collection is almost non existent. tracking and reporting for missed appointments is extremely difficult, billing secondary insurances is not an easy process, areas for notes and memos are not clean, I wish you could change appointment color based on different things such as checked in or canceled.

Wonderful product. Made my job much eaiser. Just a little more pricey than some of the other products that are the same as this.

Pros

Everything is in one easy spot. I can schedule an appointment, do telehealth, take notes, and bill all in the same platform. You don't really need anything else when using TherapyNotes, LLC. It keeps track of everything for you. Making the work load so much easier.

Cons

The price is very high. Custumer support was also hard to reach the first time I tried to contact them.

Writing case notes was something that used to take a long time and required me to create a whole new note every time. Thanks to TherapyNotes I can create and reuse a template to save time and make notes more organized and easy to read. I love the fact that the notes are all stored in one location that I can quickly access regardless of whether I’m on my desktop computer at home or through my smartphone when I’m out in the community. The software also makes it easy to filter through in order to find specific notes which is very convenient.

Pros

It makes writing case notes a breeze for mental health practitioners. It allows me to store all my case notes in one convenient place. This makes it really easy to access them whenever I need to and it also allows me to quickly filter through them to find any specific note I need to access.

Cons

The price is quite steep for solo practitioners.

I had a trial with therapy notes that was extended to 6 months. With my schedule being as busy as it is, it was hard for me to transition from my current EHR because of the steep learning curve. I found myself watching YouTube videos and their tutorials trying to figure certain things out. I did not complete onboarding because it took up too much of my time.

Pros

It is a customizable EHR which is what I was looking for.

Cons

It was hard to set up, there is a learning curve, difficult to navigate initially.
