I used Theranest for 14 years and used it for a few of those years for my growing behavioral health group practice. It served us reasonably well during an earlier stage of the practice, particularly for basic scheduling, documentation, client records, and general EHR functions. The platform is relatively easy to learn, and clinicians can usually become comfortable with the core functions without an extensive onboarding process. Though much of it is manual, and outdated. The interface had not changed much since the beginning of my subscription. For a solo clinician or smaller practice with fairly straightforward workflows, TheraNest may provide many of the basic tools needed to manage day-to-day operations, with some minor issues. Theranest has a bad reputation as an RCM and claims submission platform, it doesn't support API and lacks visibility of data. I know of fellow clinicians who lost thousands of dollars to their claims submission feature, claims got lost, some of them weren't even submitted and no way to resolve denials. Where we began running into limitations was as our organization became larger and operationally more complex. Managing a group practice with multiple clinicians, supervisors, billing staff, insurance workflows, credentialing considerations, documentation oversight, and a high volume of claims required more visibility and automation than the platform provided. As I mentioned earlier it is limited, outdated and lacks core features needed for a successful RCM. They do offer claims "support" for an extra fee, I just never trusted it. We found ourselves relying increasingly on manual processes and outside systems to monitor things such as documentation completion, supervisory workflows which is non existent, treatment plan and assessment deadlines missing, clinician performance missing, insurance and billing issues missing, and other administrative tasks. At scale, having information available somewhere in an EHR is very different from having that information organized in a way that allows an administrator to immediately see what needs attention. Billing was another area where we eventually needed greater transparency and workflow control. Our practice processes a significant volume of insurance claims, and over time we needed much stronger claim tracking, operational dashboards, automated alerts, reconciliation tools, and visibility across the revenue cycle. TheraNest was useful during one stage of our practice's growth, and I can see it continuing to work well for solo clinicians with relatively simple workflows. For a larger, insurance-heavy behavioral health organization, however, we ultimately needed a platform with deeper automation, stronger revenue-cycle visibility, more sophisticated supervision tools, and a system designed around the operational realities of managing a growing group practice.

Pros

TheraNest covers the fundamental functions of a behavioral health EHR fairly well. Scheduling, client records, clinical documentation, and telehealth. The learning curve for clinicians is also relatively manageable if you're ok with clicking "save" buttons on every page and form.

Cons

The limitations become more noticeable as a practice grows. Group-practice administration, supervision oversight, advanced billing workflows, reporting, automation, and real-time operational visibility is non existent. We frequently found that important information existed in the system but still required administrators to manually search for it, track it elsewhere, or create additional processes around it. We ended up created spreadsheets and had to hire billing staff, which defeated the purpose of purchasing a limited subscription. By the way when I de activated the clinicians in my group I was charge the full fee for the subscription, which was over $1000. They declined a reimbursement even when the platform was not being used. They also declined to help with help downloading all 2000 clinical files after over 12 years of use. They suggested these files be downloaded manually one by one, which my staff and I had to end up doing. As I mentioned earlier everything in Theranest is manual, outdated, in other words the platform has not changed much since I began the subscription back in 2014. Also, it is slow, it locks and freezes. The fees charged are just insulting for such a primitive software.

Overall, my experience has been good. I do value excellent customer service and I do feel like Theranest has this.

Pros

I sincerely appreciate the fact that Theranest makes writing treatment plans simple because of their incorporation of Wiley Treatment Planners.

Cons

I dislike the cost at times and I feel like the coast is best suited for group practices vs. solo practitioners.

Overall, the EHR helped me begin my business with a professional look and a sense of order and structure.

Pros

I like the color options on the calendar, so staff members know their app'ts without having to search. I also like the ease by which I can move between each page or tab on the screen.

Cons

The limitation of creating forms. It could be because I do not get along with technology, but I am unable to create side by side items or to add boxes for checking, or adding lines to separate sections.

I've been with TheraNest for many years. I've always love using the platform and the customer service is excellent.

Pros

The quick response from customer service. I've always had great customer service. When there is a problem or question they are always ready and willing to help.

Cons

Navigating and remembering the changes that they make. Sometimes it is difficult to keep up with the changes. However, the knowledge base is always helpful.

Overall, my experience with the TheraNest was positive. The EHR system and client portal were easy to use, reliable, and met my practice's needs. The customer support team was responsive and helpful whenever I had questions. While the billing process presented challenges, particularly regarding insurance network requirements, the overall platform and service experience were good.

Pros

What I like most about yourTheraNest is the excellent customer support. Having the convenience of speaking with a live person is incredibly valuable, as it allows me to ask questions, receive clear guidance, and have my concerns resolved quickly and efficiently. The responsiveness and willingness of your team to provide assistance have made my experience much more positive.

Cons

What I liked least about the TheraNest was learning, after several months, that the RCM service would not be able to process my insurance billing because I was out of network. I wish this had been communicated earlier, as it would have allowed me to make informed decisions and avoid unnecessary time, expense, and frustration.

It was helpful to have a colleague share their experience with me. The initiative financial referral got me hooked.

Pros

Good customer support. I can easily email them with concern and they will respond within 24 hours. I like that I can easily submit claims.

Cons

There’s a learning curve with learning different functions. In terms of self-learning to navigate the system, the learning library include other medical functions that the mental health system do not need. I feel like that was confusing when searching how to navigate the learning system. I think the cost to using the system is not easily accessible. There seems to be an additional cost to extra claim submissions, which isn’t an issue for me practice volume at this time. However, this was not clarified during the initial demo and illustration and expectation of cost for the customer.

Pros

The customer service chat and how quickly I receive a response from someone when I have a question is nice.

Cons

I have issues with clients not being able to log onto a Telehealth session. Sometimes it will say that they are connecting, or they haven't joined and they've been in the waiting room the whole time. It would be nice to have reminders about missing notes on the task page and not have to fill it out myself.

I will remain with Theranest for cost, ease of use, and the Clinical Group program. The only reason I gave 4 star instead of 5 is the NP notes but will upgrade review once this changes in the future.

Pros

Theranest has been through a lot but has finally smoothed out since Ensora aquired Theranest. I love the ease of use, they allow me to be a part of the feedback called Clinical Group where I get to test the updated applications before being rolled out to the general clinicians. I also make requests for added applications or to change applications. I love the subscription model where we pay per client group and not per individual clinicians. This has HELPED ME TREMENDOUSLY! I was with another EHR in the past and it was outrageous the price. I returned to Theranest after Ensora aquired Theranest and have not looked back. I also love that on the case notes I can modify the notes to meet my group's needs. Customer service is better today than in the past. I have not had any issues in the last 2 years with customer service.

Cons

The session/progress notes for the nurse practitioner is very limiting and did not meet our NP's needs. We had to do a lot of work arounds. This may be in the process of fixing not sure.

It has been great and easy to use and i have not had many problems and when I do they get resolved quickly.

Pros

it helps me when it comes to keeping track out Billing and Payments and my therapist busy schedules! Customer support is always super helpful when I've reached out or when there is a problem they are on it getting it fixed as quickly as they can!

I wish it would save the info automictically sometimes I get going and forget to hit save in the midst of it all and lose everything I typed in!

Overall, I had a very positive experience with TheraNest and would recommend it to therapists looking for a reliable electronic health record and practice management system. It helped me successfully grow and manage my practice for years. While I eventually outgrew it as my business model changed, I still believe it is an excellent choice for private practices that primarily provide therapy services.

Pros

TheraNest was easy to learn and gave me everything I needed to run my therapy practice efficiently. Scheduling, documentation, billing, and client management were all in one place, which made it much easier to focus on my clients instead of administrative work. As my practice grew into a group practice, it continued to be dependable and user-friendly.

Cons

As my business expanded beyond traditional therapy into coaching, education, and digital programs, I found TheraNest wasn’t built for those needs. I would have loved more flexibility with client journeys, automations, and integrations for businesses that offer both clinical services and coaching. Some workflows also felt limited compared to newer all-in-one business platforms.
