Pros: I like that our patients can fill this out in the quietness of their homes, it helps my staff prepare for a busy day by entering the patient's information ahead of time, and patients tend to be more thorough with their answers if they have time to complete the questionnaires at home. IntakeQ is always adding new features which is great too. Not all of them apply to how we use the software, however, they are always innovating and improving.

Cons: I wish the forms integrated with our EMR (electronic medical records) software so we didn't have to manually enter the data - but the functions and ability to get the patient's information before they come in outweighs this obstacle.

Pros: That it offers everything under one brand, low cost and BAA. I have been patch working a number of applications and software programs to get only a fraction of what I am getting from IntakeQ. I could not be more excited about finding them!

Cons: I think that the software could have a better layout on mobile devices or an app to use.

The customer service is outstanding. Their quick response and assistance is second to none.

Pros: The ease of obtaining important information and my clients appreciate being able to complete the forms before they come to my office. Fantastic software.

Cons: The software is expensive. I would like a plan between the Small Team and Clinic for 3 practitioners.

Overall: I use this software every day and it makes my practice run smoothly and I appreciate that it is Hipaa compliant. My clients like the opportunity to complete the forms in the comfort of their home and have it done when they come into my academy. The customer service is outstanding, especially Mike. His quick response to all of my questions and requests is far beyond any other company I have dealt with over the last 10 years.

The software has been very useful for my outpatient practice.

Pros: It appears to be a perfect balance of user friendliness with complex features. I have an outpatient practice where it is important to get guardian signatures. Compared to competitors intakeQ's customization is far superior.

Cons: It so attractive to use, I want to spend all my time on it, but I have other things to do so that is challenging.

Overall: Streamlining intake, followup and care delivery processes in a setting where I do not have admin assistant. IntakeQ is my admin support.

Use for online intakes for my counseling practice. User-friendly for clients and for me.

Pros: Easy to send link to clients. Forms and site are professional and customized to my practice, as well as easy to change. Customer service team has been very helpful when needed. Nice that subscription is scalable for my small practice.

Cons: It's not integrated with my practice software, so I have to manually enter data from intakes into client charts.

Fantastic for what we needed!

Pros: I love how easy it is to make my own forms and templates. I love the affordable price as well. Intake Q is the best Stand alone Software I could find after 4 years of research. It is simple practical and makes so much sense. I have zero programming experience but knew this was exactly what we needed after just a few days on the free trial mode! I signed up before the free trial was even over. Also I love how they have actually taken some of my suggestions and made them features! Bravo!

Cons: I wish there were a few additional features such as if a table equaled a score that the score could mean something. Also a better way for clients and provider to use the same form (kisok mode??) And allow for color customization as well for the side bar where they are just blue and green right now. Oh and a little more security on who can delete what. that would be good.

Overall: this software has helped us become a paperless office!

Pros: very easy and quick to send paperwork to patients, patients also seem to like the convenience and ease

Cons: once a patient starts their paperwork and doesn't finish, they don't seem to go back to finish...not sure if we need to resend it or if its just a patient error

Great customer service! Great functions but wish I could receive faxes as well as send them.

Pros: The functions and convenience~ I really like being able to send my intake packet on-line. My clients love this also because they are used doing things on-line. It is so helpful because I usually get the packet completed and returned the same day I send. It allows me time to prepare for a new client. I like NOT having paper and instead of having a comprehensive electronic client file. The appointment reminder for a first appointment is wonderful!

Cons: Not user friendly and not very intuitive. Great customer service but I would like to be able to do things without having to contact customer service! I would like to be able to click on a current form to see out something was created so I could then do it myself on other forms . I find my self looking at a form that was done for me my customer service and wondering "Now how did they do that?"

I really dislike that I can send but not receive faxes -- how great would this be to receive a client fax and be able easily to attach it to that file! I would even pay extra for that function,

We are quite satisfied with IntakeQ platform: it is reliable, convenient and simple to use.

Pros: The crucial thing for us was it to be an HIPAA secure platform (we use it to collect some sensitive (medical) information for a research study). It has exceeded our expectations regarding 1) reliability (the platform works smoothly), 2) simplicity (easy to user - on both ends) and 3) great customer support (we had ran into couple problems that were fixed pretty quickly). Also, the messaging was a bonus feature for us - so far we used it for a few occassions but they were very critical so it was great to have that option. And the price is reasonable - we are an independent research team - paying for this service out of pocket - and we are extremely satisfied with every penny we pay to your service. So, great job to you gals & guys at IntakeQ!

Cons: The questionnaires can be improved:
- such as by adding answer-activated conditional questions , ex: if you check a box at Question1, then skip to Question8 etc.)
The display can be improved by having options
- such as by adding the option of showing the most recently completed form on top
- visually grouping forms - as worked on etc (not just read or unread) by user-activated flagging.

Overall: HIPAA secure platform (it was a must for our IRB approval)

Pros: I love that clients can schedule their own appointments, get email/text reminders, and fill out paperwork online.

Cons: I wish there were more features available for linked calendars and spacing out appointment times. For example, I have it set within IntakeQ to link with my work Google calendar. When a client schedules an appointment through IntakeQ there is a 15-minute space that is created between sessions. However, sometimes clients schedule an appointment with me which I put into my Google calendar. These appointments are not scheduled through IntakeQ so, therefore, there is not a 15-minute space between sessions with clients. As a result, I'm often left with back to back appointments with clients without any break. I realize I can go into IntakeQ and schedule the appointment however that is not always possible.

Overall: Having clients be able to schedule their own appointments and access paperwork online is a huge benefit. I also love that clients get a text reminder, which means I don't have to remind clients of their appointment time.

We love this program! It's made our front desk's job so much easier. Great customer service!

Pros: Super fast customer service response time. The software itself is very easy to use. All of our patients have really loved it.

Cons: It was hard to change the login and admin area. I had to contact customer service to be able to get in and make changes.

Overall: Less staff time on manually entering paperwork.

I like the tool but need more flexibility and workflows to support my business.

Pros: Hippa compliance is the primary driver for the why we moved to the SW as well as being able to have clients complete forms online. I like the professional look and feel. The search is helpful.

Cons: Not enough flexibility for processing intakes in bulk. Would like the ability to transfer in bulk between providers, link custom template office use fields with client custom fields so certain notes are pulled into the client's record. We need more flexibility in configuring the SW to meet our business needs. We have to use a separate scheduling SW because intakeq doesn't offer the same flexibility in setting up different classes. We use acuity and perhaps I just don't know how to effectively use the booking capability in intakeq. It would be really helpful to have workflows that you could define for intakes. For example, if a new intake comes in, I could set the default status to pending and on the dashboard only show pending intakes. Once the office processes the intake I could define a new status, for instance, ready or action-required... We would need the ability to display and filter based on status. It would be helpful to allow flexibility in defining any number of workflows. Unfortunately, given our business, I have to build manual processes around the SW even though we're doing very standard things. One other thing I've noticed is that I can only have about 5-7 open intakes at once otherwise the application hangs.

Pros: Being able to set things up and send quickly to clients. I really like the online booking calendar, this has helped me a ton! I have not hooked it up to my website yet, but plan on doing that soon. Has definitely helped me streamline my business. I still need some work with notes and that kind of thing.

Cons: Just wish I could email my info to them in a better manner. Still have to figure that out. I have not used customer service, yet....to be determined

Overall: Easy to use. Streamline what I use for forms. Easy for people to fill out. I like how it saves as they go. Love online booking. Still working on the package thing on my end.

Pros: I love how easy it is to create an intake form and consent forms. There is a wide selection of question types and it is VERY easy to edit.

Cons: I don't like that there is No spellcheck feature. Another problem I found that it is too easy to delete a form accidentally.

Pros: I like that we can edit the intakes once they come in and use them to write our initial assessment notes.

Cons: I wish that it function more like a full fledged EHR- having options to bill using CPT codes, would be great, for example.

Pros: I like the ease of use and the flexibility it allows us to have with scheduling our patients. The patients love the fact they receive their intake forms electronically before they arrive at the office. They also love the fact they get text reminders from us. This has helped us minimize no-shows.

Cons: I haven't been able to override blocks in the patient schedule without removing or editing the blocks.

Pros: It does everything you need, it's very simple to use, has a great mobile version of the website, and is intuitive.

Cons: Sometimes it can be glitchy and not send appt confirmations. Doesn't allow you to automate sending consent forms in emailing automation. Would love an app!

Pros: The form workflow works well for us. I have 2 kiosk computers running Porteus Kiosk (a linux distro)

Cons: I've heard complaints from our team that customer/tech support hasn't been very helpful or were unable to understand their issues. For some reason, our assistant account needs password resets and it's not a simple process.

Here's my feature wishlist:

Spanish versions of everything (Some of our Spanish speaking patients would like to see even the button text in spanish)

Wizard driven, step by step, form creation.

Icons that feature the form

Allow multiple administrators or power users per account so that we can fully audit access to patient info.

Overall: Saves paper, patient time, and front desk time.

Pros: We like everything about the software. It's incredibly easy to use for staff and for patients. It's also very easy to customize forms. Support is fabulous and helps immediately with any questions or issues.

Pros: I love that I can create documents. I also love that clients can sign them electronically. Overall, its an amazing client tracking system.

Cons: The think I dislike is that there is really no way of tracking client payments and invoices. It would be nice to have those features.

Your customer service is excellent. I have been dealing with rep who is quick and very helpful.

Pros: Using this software allowed our customers to quickly access and submit a referral, which increased our client numbers. We were able to tweak our intake forms many, many times. Now it's working. We have had a positive experience using IntakeQ.

Pros: -Ease to create forms
-I can use the notes to track all of my daily notes with patients -- no need to buy a separate EMR system.
-Easy for patients to schedule appointments
-Excellent customer service whenever I have an issue
-Constantly updating and improving with new features.

Cons: -The booking settings can be difficult to figure out and manage
-The system isn't great for managing more than one practitioner. I run my practice with my partner, and it would be nice for us both to have full admin privileges. Likewise, I get email notifications about all of her appointments, which is unnecessary.

Overall: Was able to start my practice quickly and without a major investment. I can get up and running with professional intake forms and scheduling, and seamlessly write electronic notes for all of my patients. It is truly excellent.

Pros: Paperless, able to complete their file before they get here vs. waiting on them and they do not have their paperwork. Shortens wait time and speeds our process.

Cons: Receiving a notification on our desktop like Solution Reach, rather than checking our email frequently.

Overall: It is extremely time saving. Being a small but busy office, it takes the burden off of us when they get here, of having to go over new patient paperwork. It is user friendly and for all ages.

Pros: So easy to use and set up our forms. Super customizable. Our patients love that they don't have to print out paperwork or come early to their appointments. I would highly recommend!

Cons: mac users seem to have a hard time submitting forms. Patients often complain of getting stuck and being unable to submit completed forms.

Pros: Unbelievably easy to send and receive highly confidential documents. Clients LOVE that they don't have to print or scan anything! Customer service is top notch and every time I've needed to change or update my forms they've been super fast getting things done. LOVE this company- Ive saved lHOURS every week using this and now my office is totally paperless!

Cons: I wish they would include progress notes and scheduling. I use another online program for all of that- but if and when intakeQ adds it I'll definitely be switching!

Overall: Ease of use, transitioned to paperless office. Clients love being able to do intake paperwork online.

Pros: I love the ability to modify and create the intake forms, the online booking is easy, and the automated emails are great.

Cons: Customers have difficulty when they don't complete a form going back to it, not sure why. I'd like to make charges not attached to an appointment but that's not that big of a deal. The thing I would change and it's super tiny, the manage appointment link is not very visible and people don't realize they can cancel/reschedule etc.

IntakeQ has been an excellent service for our practice! I would highly recommend it to others!

Pros: The ease of use is great. Our patients have not had any issue completing the forms and the customer service has been very good.

Cons: It was slightly difficult to set up a payment plan due to the fact that I am in the United States and my bank would not allow me to use my card.

Cons: I have to type in the client's name to retrieve their notes. It's a hassle having to click back and forth between "Booking" and "Clients"

Pros: My clients love being able to fill out their paperwork on their smartphone, tablet or laptop. I love knowing that once I electronically sign the forms they are securely and confidentially stored. They were so helpful in getting my forms set up and even inputted them for me from a PDF. The price is very reasonable and is more that offset by the time I save if I had to handle printouts, scan and then file forms in compliance with HIPAA requirements.

Cons: The scheduling software is nice, but the scheduler doesn't commnicate with my google calendar or even the calendar in my practice management software.

Overall: This software saves me time and makes it easy to retrieve contact information and consent forms for my patients.

Pros: It so easy to use intakeq. They have made navigating through the site so easy. I would definitely recommend.

Pros: The software is easy to use and to modify. I am happy to be able to download and upload the completed forms to save in our EMR. >90% of our clients appreciate the simplicity and ease.

Cons: Occasionally it will give users feedback that the form is incomplete and can be confusing for them. Sometimes the email goes to their spam and they miss the email and we need to remind them.

Pros: User friendly with excellent customer support. No more paper promotes conservation and simply allows me to transfer my intakes to my progress notes software.

Cons: Nothing significant but sometimes my lack of computer skills makes it difficult for me to make changes to forms.

Super easy, clean, enhanced, affordable.. all with superior customer service. Very hard to beat!

Pros: Excellent forms creations, including the ability to electronically send & receive to client. And with that, so many options too, such as sending a link by email or having it embedded right on your website. But it doesn't stop there, their amazing online booking program works flawless. And third, the ability to store securely client notes, progress charts and more. It's like combining all 3 critical aspects of running a private practice into one program. And the price is fair. All this from a Canadian company makes it unbeatable!

Overall: In the world of practice management software and ancillary systems, it is a rarity to find a company like Intakeq. Not only was I able to set up all the forms, the way I wanted them, with the nomenclature as I need it, but if I hit a snag, I sent a note to support, who sometimes send me an idea to help me, but 9 times out of 10 simply made the correction or change for me. Not enough ways to praise this company.

Overall: They combine secure forms with a great scheduling interface. Upgrades and new features continue to be added. IntakeQ makes scheduling and forms very simple yet customizable to the individual. Their customer support is immediate and efficient and friendly.

Overall: I cannot rate this high enough. We used Docusign for years but there were always issues and it was very cumbersome doing most of the work yourself. We searched for an alternative online Signature site and there were so many. However, IntakeQ was the most superior of them all. At first I was worried about no phone support but let me tell you, that has NOT been an issue at all. As soon as you email anything, within mins - sometimes seconds - you get a reply. It is awesome and they are SOOOOOOO helpful and so eager and willing to assist with any questions. The best part is they do most of the work for you YAY. I can't say enough good things about them, they truly are a lifesaver. Plus it is so simple to use and they can customize things to suit your needs which makes them much more flexible than any of the other providers. Do yourselves a favor and look no further. This is the company you want to sign up with hands down!!!!

Overall: I am fairly new to IntakeQ but personally, think every office should have this. It makes our office eco-friendly and also makes the transition of things go faster in our office. In this day and age, most people type faster than write. It allows the pts to look at frames and everything instead of filling out silly paperwork before the appt. It makes our office stand out.

Cons: Clients have had trouble using it the screen freezes on mobile devices and they are unable to book an appointment

Overall: It took a while a lot longer than expected to add to the website and it still freezes to make appointments on mobile devices but the price is good and it seems to be working reasonably well so far

Pros: The convenience of it has made going paperless fairly easy.

Cons: It does not work well with out Tablets and Ipads.

Overall: I run and manage a high volume chiropractic and wellness office. We see 350+ patient visits per week and in May of this year, we switched to a paperless office. Intake Q made the transition bearable. Customer support was very helpful in editing our forms and went above and beyond to assist us. Now if only Intake Q offered an app it would make using your services with an IPAD or Tablet so much easier. Also making it so we can send the signer a password would be helpful as well other than that I Love INTAKE Q and tell everyone who wants to switch to paperless to give them a try.

Pros: Simple to complete. It has all the features that are needed for a company needing to stay HIPAA compliant in a secure manner. Lot's of good features to compliment the form software (i.e. Scheduling, etc.)

Cons: It would be great if it had a point of sale aspect as well as a feature to hold electronic medical records (similar to practice fusion). If those two systems were able to be combined, it would be the perfect system for our business.

Overall: Intake Q has streamlined our New client paperwork to a simple, online form that can be completed and submitted back within minutes of a patient being seen. This saves us a ton of time and money (no need for paper forms anymore) and we're 100% satisfied with this. The fact that it's able to be placed on our website is another bonus. Comments: I am a single, one-person office mental health provider. IntakeQ has really helped to streamline my patient scheduling and patient intake forms process. My patients love the email appointment confirmatioms and automated text reminders that are available to use. I am very pleased with the software and the avaialbel support. The only thing that would be nice would be to have some phone support-a number to call if help is needed, but I have found that when I email I get a response back very quickly, so overall support IS good.

Comments: IntakeQ has revolutionized our intake process. It has dramatically reduced the amount of time our office staff spends on the phone with potential new clients because we are now able to instruct our clients to provide all the necessary information via the secure, HIPAA compliant IntakeQ form. We also have been able to streamline our assessment process and shave a full 30 minutes off the appointment time due to having the consent forms and background information forms already completed before the appointment. The additional feature of being able to download completed forms from IntakeQ in PDF format allows us to easily upload the forms into our practice management system.

IntakeQ should be a no-brainer for ANYONE in the field of mental health!

Comments: I have been using IntakeQ for about 6 months. In general, it gets the job done. I have a few needs within my psychotherapy practice that it doesn't meet: 1) I can not create a document with two separate signature fields (i.e. when 2 partners in a couple want to sign a release of information). This is a recurring need so I find myself frustrated with having to create two documents for them. 2) you can't partially fill out a form for a client before sending it to them. This often requires I send a separate email explaining to clients how to fill out 'my part'. 3) Customer service is all by email. This can make for a lengthy trouble shooting process when you really need them. I found out they will schedule a time to help you if you schedule in advance however my 'issues' have popped up last minute and it's when I don't have a phone number to call. The employees are VERY nice and helpful when they do communicate via email.

Overall: Before online forms for our office we spent hours on end scanning, uploading, and shredding office intake forms. NOW we don't waste paper and forms are filled out more in depth than the paper kind. Highly recommend for the money you save and spend on the forms.

Cons: Cant use the same email address for more than one person. e.g.. when someone forgets to fill out a form and I send it to the office email and then give them the iPad.

Overall: SO easy peasy, don't have to teach the staff, its just apparent

Would be awesome to have an automatic send option when a client books an appointment from our booking website

Overall: There are so many options out there. I am SOOO glad I found intakeQ. It makes my day go much more smoothly. My patients are able to complete and sign all my intake forms and consent forms. I can reject an incomplete intake and ask the patient to complete it. No paper is involved. I also use intakeQ to accept payment for my services.
Customer service is the best of any software I use. They graciously listen to my recommendations to improve my work flow and are very professional.
I recommend intakeQ to everyone who will listen.

Comments: I heard about IntakeQ from another holistic practitioner. I tried to create my own questionnaires but it was far easier to pay extra for IntakeQ to create and customize them for me, with my own logo. Sometimes my clients complain of errors and difficulty submitting their responses, which is frustrating for them and me, especially when I find them complete in the "Not Completed" file. Even when there have been problems, the customer service is prompt and friendly. I have not integrated the schedule into my website, so I cannot comment on that.

Recommendations to other buyers: You should be aware that there will be an additional foreign transaction fee if you are a US-based user. Overall, it's satisfactory but could be improved.

Pros: I like how easy it is to create the intake form and how easy it is to edit it.

Cons: I have had clients complain that they lose their password, or they log it from a different link (because sometimes I send two or three emails before they respond) and then their data isn't there. It seems that if they type an email that has already been entered into the system that they should be prompted to re-enter a form they already started or prompted with a password reminder

Overall: For years we sent out questionnaires to learn about our clients via email with an attached PDF. It was not very user-friendly. The client had to either print it out and handwrite their answers or try to edit it digitally which often resulted in lost data. When we switched to IntakeQ it simplified the process for our clients and almost immediately, we noticed an increase in response time. IntakeQ is user-friendly, so even our older clients, who may not be as comfortable using a computer, seem to have an easy time filling out our survey. We switched over six months ago and will never go back to our old system.

Pros: I like that once it is set up I can just send it out and not worry about it. I also like that I can see it instantly once a client fills it out.

Cons: Difficult at moments to set up. Not everyone receives the email.

Overall: Overall, I am happy to have found IntakeQ. I have had a little difficulty with a few things while setting up my survey, because I am not a very tech-savvy person. But I was able to make it work. I compared prices of all of these types of services and this one was the best. I do a lot of online marketing and health surveys are a good way to filter out those with a dis-genuine desire for the work (I'm a female massage therapist). I would recommend this to friends and family, particularly those in the massage field.

Recommendations to other buyers: If you're not sure what to put in your intake (I wasn't), google various intakes and combine the best aspects of what you find. Explain to clients that your thorough nature is to ensure the best service and that it will only need to be done once. When you're done, send it to yourself and time yourself so that you can say it will take __(your time)__ to __(your time plus 5 minutes)__ to do. This will also allow you to see what they see. I have referred back to my intake more than once.

Pros: Ease of use, all in one place, very flexible

Cons: EHR could be more developed to make it alternative to paper.

Overall: I have been using intakeQ for a bit more than a year now and I am very satisfied. Whenever there was a glitch or question they were quick to respond and resolve. My patients like it. I love that many needed things are available in one place - online scheduling, reminders, intake forms, a place for patient notes, and now also a way to track basic financials. Love it!

Comments: We've been using Intake IQ for less than a year and use it exclusively for sending out intake forms. My one complaint is that patients sometimes can't view the entire form on their phones. No issues with computers but seems to have a few glitches with mobile devices.

Pros: Visual appeal. Easy to complete forms for my patients. Easy for me to edit on the fly.

Cons: No faxing feature yet (that I'm aware of...)

Overall: Visually appealing and professional forms. Easy to create and edit. Excellent customer service with very quick turn arounds.

Recommendations to other buyers: Please add a fax line.

Pros: I love the automated forms - so easy to use and update.

Overall: We use Intake Q to manage all of our patient forms - that alone has saved us a ton of admin time and effort. The schedule and billing are easy to, but the forms make all the difference to our patients and providers - this is a tool that we could not run our business without!

Recommendations to other buyers: I wish it were easier to embed on our website - it's probably me being the holdup, but it's something on my to do list.

Pros: All online. No paper to waste or keep track of.

Cons: I would love for it to be less expensive.

Overall: We began using IntakeQ at our spa in April 2016 for clients completing their questionnaires & waivers for their scheduled services. They were so helpful with getting me on board - answered all of my questions & quickly converted my original documents to the software. We were up and running in a couple days. It was a very smooth transition to paperless for staff and clients. It makes the intake process more simple while more sophisticated. A great enhancement to our spa!

Pros: Patients to book online, online forms, use from multiple computers, customization settings

Cons: We'd like to be able to customize more settings, mainly that we can color-code appointments on the booking page.

Overall: There are a lot of great features and has made scheduling so much better for our office. So glad to have gotten rid of the paper version of the schedule. We love the option for paperwork to be completed online as well as other forms. There have been a few glitches and some features we wish were available. We love seeing what updates come out each month.

Recommendations to other buyers: We'd like to be able to customize more settings, mainly that we can color-code appointments on the booking page. Basically, to indicate at a glance if everything is ready ex. could be green prior to the appointment and ex. yellow if records are needed to be received prior to exam or blue to still verify insurance. We also would like more space in the notes section on the bookings window. It would be nice if on the bookings window how there is a feature to "add notes" it's very helpful to also add a quick link to go to "overview" or profile. It would just be a lot faster than having to click add a note then cancel to see their overview/ profile or re-typing the name at the top to search clients.

Cons: Not really functional for practitioner soaps and insurance paperwork.

Overall: Booking calendar easy to use. Patient reminders are consistent and email notifications for online bookings are prompt.

Comments: We see insured patients and have our own EMR and scheduling system. IntakeQ is helpful for improving our efforts to be paperless, reduce written errors, and decrease time patients spend in the waiting room. We would love to electronically integrate IntakeQ somehow to our patient charts on EMR in the future.

Comments: My experience with Intake Q has been very positive, it's been about 2 years now. I love the ease of creating and editing intake forms and bill of rights. I also live the appointment booking software, it has really helped me cut down the number of no-shows! Makes my work much easier. Customer support has always been great too whenever I have had a question. I highly recommend the software.

Comments: It is so nice to be able to direct my clients to a website to complete their paperwork. I built the forms myself. Anytime I needed help I got a quick email, and always an offer to correct the problem for me if I was unsure. I don't have to handle paper, or scan or file anything. Overall: I previously used multiple systems to manage my client calendar, payments processing, and intake forms. I stopped paying for the others because IntakeQ keeps getting better and better so the others are now unnecessary. My favorite new feature of IntakeQ is the ability to send secure documents to clients.

Overall: I came across IntakeQ upon recommendation from a colleague. At first, I didn't really see how it could possibly have forms that just like what I'm used to...until I sent over my form for them to transfer over. It was done so quickly! And it looked great! I had a beta group of patients to try it out first and they complimented immediately on the ease of use. Whenever I come up with an idea or think I need a different feature, they either already had it (and I didn't know it yet!) or they provided another solution until it could get added. Amazing people!

Cons: No way to embed forms into the website - available through link only. We would love a feature to embed the full form onto a web page or secure patient portal without going through an IntakeQ page.

Pros: Very intuitive with cleanly designed forms and easy managing of completed paperwork.

Cons: I wish that either they offered a full package to run a private practice, or made the forms and other items available at separate price points since I only use the forms.

Overall: I use this software for the paperwork in my private practice as a psychologist. A very easy form building and usage system. There are pre- built templates and questions that can be used and a very intuitive form building process. Terrific help from support converting some of my existing paperwork. Super helpful and reasonably priced. The workflow is a dream with client information and it is all HIPAA compliant. There are other scheduling and note feature included that I do not use since I have other software for that, but it's well worth it just for the form capabilities. You won't find anything else out there that is this good at this price point. Bravo!

Recommendations to other buyers: Shop and compare, but I doubt you'll find an HIPAA compliant form management system that's this inexpensive let alone this easy to use.

Pros: Its security and ability to design my own intake questionnaires.

Cons: Some of the functionality, inability to organize files, etc. I also wish the set up for the consent forms was easier, i.e. being able to add drop downs and multiple choice, multiple entry types of questions on them.

Overall: I really like and appreciate this service overall. Where I think that it can improve its functionality is the ability for the user to organize and categorize their questionnaires.

Overall: We started using IntakeQ a few months ago and are very happy with the product. Our patients love being able to complete forms online. Customer Service was incredibly helpful in setting up our initial forms for us and was very quick to respond whenever we needed a change or had a hiccup. My only regret is that we didn't know about IntakeQ a long time ago!

Recommendations to other buyers: Exceptional product for patients to use to fill out forms ahead of time.

Cons: Support does not respond, though they do fix the issue

Overall: IntakeQ has made creating intakes for our clients and different treatments easy. Customer support has fixed any issues we have had quickly, though they just fix them and do not call us or email us back to let us know it is fixed. It just works.
I like that there is a number of already made forms so I can alter those questions as I need to without having to start from scratch. I like that I can open the forms in the office or send one to the client beforehand depending on how extensive the forms are.

Pros: easy to use, make changes and send to patients

Cons: the printable version is very long; >20 pages, could be much less if it weren't so spread out in print version.

Overall: This has been a huge time and paper saver for me. Most of my patients love it. Some people that are still uncomfortable with doing online forms can get baffled and occasionally people have a challenge with saving the information (maybe it's their browser?). I also like how easy it is to make changes on the forms.
My only recommendation is to condense the printable form. My health history is >20 pages printed. Probably could be 1/2 that if it wasn't so spread out.

You can find it in the Settings tab under More -> Settings. You should enable both options in the PDF section. I hope this helps!

Cons: -Wish there was a feature to have clients put things in chronological order like some other Functional Medicine Software companies.

Overall: WOW! The ease of use is crazy simple....and if you get stuck-- if you send an email to support, you WILL hear back from a rep within the day, and I mean if you send it at 7pm you will get an email back at 8pm. yep even after hours. I secretly want to hire their staff because their customer service is top notch. But not only is the staff amazing, the product is THE BEST solution for my functional medicine practice. HIPAA compliant. Getting started with them-- they were able to convert my current forms into digital questions for a mere $9 or something like that. I would've wasted HOURS trying to figure that out. If you are considering intakeQ, look no further. I would do a video testimony for them because I have been so impressed. (and no I don't work for them)
