Pros: Hippa compliance is the primary driver for the why we moved to the SW as well as being able to have clients complete forms online. I like the professional look and feel. The search is helpful.

Cons: Not enough flexibility for processing intakes in bulk. Would like the ability to transfer in bulk between providers, link custom template office use fields with client custom fields so certain notes are pulled into the client's record. We need more flexibility in configuring the SW to meet our business needs. We have to use a separate scheduling SW because intakeq doesn't offer the same flexibility in setting up different classes. We use acuity and perhaps I just don't know how to effectively use the booking capability in intakeq. It would be really helpful to have workflows that you could define for intakes. For example, if a new intake comes in, I could set the default status to pending and on the dashboard only show pending intakes. Once the office processes the intake I could define a new status, for instance, ready or action-required... We would need the ability to display and filter based on status. It would be helpful to allow flexibility in defining any number of workflows. Unfortunately, given our business, I have to build manual processes around the SW even though we're doing very standard things. One other thing I've noticed is that I can only have about 5-7 open intakes at once otherwise the application hangs.

You may want to look into our client tags feature, which can be automated and allows you to filter by tags later. Pros: Initially found the customer service to be exceedingly prompt and abundantly proficient. They set up cost for the forms is more than reasonable and the monthly expense is such a bargain. I have never known their system to go down. Simple to get going and no maintenance. We keep finding new uses for them. It is so great to be able to email a link to the client. The client follows the link, fills out the form(s) and then submits. The forms are easily downloaded. Could not be more pleased. Thank you, thank you, thank you...

Cons: Seriously, I have yet to find one. I truly wish my every interaction could be an interaction like I've had with IntakeQ!

Overall: Tremendous ease! I send out the forms and they get filled out. Not copying of forms. No storing of forms. No purchasing paper and toner. Fewer calls to the copier repair guy. They make my life better...

Pros: Well designed, well coded. Visually pleasing. Constant improvement and addition of new features. Excellent support.

Cons: I have no significant cons about this software. It is excellent.

Overall: I've been with IntakeQ since the early days. Even from the very beginning, it was a solid product with excellent pricing. I have noticed over time that new and very useful features are added on a consistent and frequent basis, which I love. This service is continually improving and continually impressing me. It has been an important and welcome addition to my medical practice. I fully endorse IntakeQ!

Recommendations to other buyers: I really like the continuous addition of new features, and the pricing is excellent. I think the addition of the analytics feature is HUGE and could generate a ton of new business for IntakeQ for research purposes. I am grateful for Cassio's immediate and positive support through the time I've used it.

Cons: Sometimes hard for patients to log in (though most of these cases are technically impaired folks!)

Overall: I initially stumbled onto IntakeQ when I was searching for a good, cost effective way of handling my medical paperwork. As it turns out, this was the most beneficial "stumble" I could have done. My intake process is now streamlined and efficient. Creating new forms is a breeze. In the few cases I've had problems, the team at IntakeQ has quickly and effectively come to my assistance, even so far as to create the forms I was needing. Anytime I have a question I get a prompt personal response. These guys just keep improving the product more and more. I highly recommend them, particularly if you are a solo doc like me. But even a large group would benefit. High-quality product, HIPAA secured, easy to use, responsive team, very affordable.

Cons: -Wish there was a feature to have clients put things in chronological order like some other Functional Medicine Software companies.

Overall: WOW! The ease of use is crazy simple....and if you get stuck-- if you send an email to support, you WILL hear back from a rep within the day, and I mean if you send it at 7pm you will get an email back at 8pm. yep even after hours. I secretly want to hire their staff because their customer service is top notch. But not only is the staff amazing, the product is THE BEST solution for my functional medicine practice. HIPAA compliant. Getting started with them-- they were able to convert my current forms into digital questions for a mere $9 or something like that. I would've wasted HOURS trying to figure that out. If you are considering intakeQ, look no further. I would do a video testimony for them because I have been so impressed. (and no I don't work for them)

IntakeQ has made scheduling, sending forms, and reminding clients of appointments much easier.

Pros: I love that clients can schedule their own appointments, get email/text reminders, and fill out paperwork online.

Cons: I wish there were more features available for linked calendars and spacing out appointment times. For example, I have it set within IntakeQ to link with my work Google calendar. When a client schedules an appointment through IntakeQ there is a 15-minute space that is created between sessions. However, sometimes clients schedule an appointment with me which I put into my Google calendar. These appointments are not scheduled through IntakeQ so, therefore, there is not a 15-minute space between sessions with clients. As a result, I'm often left with back to back appointments with clients without any break. I realize I can go into IntakeQ and schedule the appointment however that is not always possible.

Overall: Having clients be able to schedule their own appointments and access paperwork online is a huge benefit. I also love that clients get a text reminder, which means I don't have to remind clients of their appointment time.

We are quite satisfied with IntakeQ platform: it is reliable, convenient and simple to use.

Pros: The crucial thing for us was it to be an HIPAA secure platform (we use it to collect some sensitive (medical) information for a research study). It has exceeded our expectations regarding 1) reliability (the platform works smoothly), 2) simplicity (easy to user - on both ends) and 3) great customer support (we had ran into couple problems that were fixed pretty quickly). Also, the messaging was a bonus feature for us - so far we used it for a few occassions but they were very critical so it was great to have that option. And the price is reasonable - we are an independent research team - paying for this service out of pocket - and we are extremely satisfied with every penny we pay to your service. So, great job to you gals & guys at IntakeQ!

Cons: The questionnaires can be improved:

- such as by adding answer-activated conditional questions , ex: if you check a box at Question1, then skip to Question8 etc.)

The display can be improved by having options

- such as by adding the option of showing the most recently completed form on top

- visually grouping forms - as worked on etc (not just read or unread) by user-activated flagging.

Overall: HIPAA secure platform (it was a must for our IRB approval)

Pros: All the scheduling features, analytics, and customizable forms.

Cons: Still looking for more automated email functions, and more scheduling features, but IntakeQ team seems to always be doing updates that are great.

Overall: I can't say enough good things about how amazing IntakeQ is for me as an independent contractor and licensed massage therapist. IntakeQ has taken so much of the burden of administrative duties off of my plate. All of my health intake forms, waivers, and general information pamphlets are all customized and easily sent to clients. The scheduling feature and integration with gmail is awesome for both me and my family who need to know my schedule. Email features, integration with Square (for standard sales) and Stripe (for gift certificates) is great. This is one powerful tool for HIPPA compliant forms and administrative needs. couldn't survive without it.

Recommendations to other buyers: Have your team look into FullSlate, maybe set up a test account, and integrate some of the automated emails, and other client features.

Pros: easy to use, make changes and send to patients

Cons: the printable version is very long; >20 pages, could be much less if it weren't so spread out in print version.

Overall: This has been a huge time and paper saver for me. Most of my patients love it. Some people that are still uncomfortable with doing online forms can get baffled and occasionally people have a challenge with saving the information (maybe it's their browser?). I also like how easy it is to make changes on the forms.

My only recommendation is to condense the printable form. My health history is >20 pages printed. Probably could be 1/2 that if it wasn't so spread out.

Thank you so much for the positive review! Regarding the issue with printed forms using up too much paper, you can enable a more condensed version that should work better for you. You can find it in the Settings tab under More -> Settings. You should enable both options in the PDF section.

Pros: I like how easy it is to create the intake form and how easy it is to edit it.

Cons: I have had clients complain that they lose their password, or they log it from a different link (because sometimes I send two or three emails before they respond) and then their data isn't there. It seems that if they type an email that has already been entered into the system that they should be prompted to re-enter a form they already started or prompted with a password reminder

Overall: For years we sent out questionnaires to learn about our clients via email with an attached PDF. It was not very user-friendly. The client had to either print it out and handwrite their answers or try to edit it digitally which often resulted in lost data. When we switched to IntakeQ it simplified the process for our clients and almost immediately, we noticed an increase in response time. IntakeQ is user-friendly, so even our older clients, who may not be as comfortable using a computer, seem to have an easy time filling out our survey. We switched over six months ago and will never go back to our old system.

Overall: we are using IntakeQ for our intake paperwork only. We are definitely seeing an increase in clients filling out paperwork before their first appointment. I like the new features added especially the analytics. We have had a few glitches with some clients not being able to submit their paperwork and there really was never a resolution. It may have been an issue with their own computer. overall we have been very pleased with the way things have been going. I have been able to make changes to documents myself and that has been very helpful. We have added staff and I have added names. I have had to add options to documents and the interface has been very user-friendly. Customer service has been very helpful and responsive in reopening a clients file so they could finish filling out their paperwork.

Pros: I like that our patients can fill this out in the quietness of their homes, it helps my staff prepare for a busy day by entering the patient's information ahead of time, and patients tend to be more thorough with their answers if they have time to complete the questionnaires at home. IntakeQ is always adding new features which is great too. Not all of them apply to how we use the software, however, they are always innovating and improving.

Cons: I wish the forms integrated with our EMR (electronic medical records) software so we didn't have to manually enter the data - but the functions and ability to get the patient's information before they come in outweighs this obstacle.

Pros: My clients love being able to fill out their paperwork on their smartphone, tablet or laptop. I love knowing that once I electronically sign the forms they are securely and confidentially stored. They were so helpful in getting my forms set up and even inputted them for me from a PDF. The price is very reasonable and is more that offset by the time I save if I had to handle printouts, scan and then file forms in compliance with HIPAA requirements.

Cons: The scheduling software is nice, but the scheduler doesn't commnicate with my google calendar or even the calendar in my practice management software.

Overall: This software saves me time and makes it easy to retrieve contact information and consent forms for my patients.

Pros: Easy to use. Easy to set up fairly complicated intake form rules and booking settings. Digitizing our intake forms and booking in general has definitely streamlined both processes for our business.

Cons: Lack of family support - e.g. a parent booking appointments for multiple children has to use several different email addresses, and the profiles often fail to "link" properly so we're constantly merging client profiles. Lack of integration is a huge con for us - we have to update our EMR schedule with booking changes manually, payment info doesn't link to our billing vendor (our Google calendar does synch - hoping for more future integrations).

Overall IntakeQ is great. It's allowed me to successfully start my own physical therapy practice

Pros: -Ease to create forms

-I can use the notes to track all of my daily notes with patients -- no need to buy a separate EMR system.

-Easy for patients to schedule appointments

-Excellent customer service whenever I have an issue

-Constantly updating and improving with new features.

Cons: -The booking settings can be difficult to figure out and manage

-The system isn't great for managing more than one practitioner. I run my practice with my partner, and it would be nice for us both to have full admin privileges. Likewise, I get email notifications about all of her appointments, which is unnecessary.

Overall: Was able to start my practice quickly and without a major investment. I can get up and running with professional intake forms and scheduling, and seamlessly write electronic notes for all of my patients. It is truly excellent.

Cons: Support does not respond, though they do fix the issue

Overall: IntakeQ has made creating intakes for our clients and different treatments easy. Customer support has fixed any issues we have had quickly, though they just fix them and do not call us or email us back to let us know it is fixed. It just works.

I like that there is a number of already made forms so I can alter those questions as I need to without having to start from scratch. I like that I can open the forms in the office or send one to the client beforehand depending on how extensive the forms are.

Pros: Simple to complete. It has all the features that are needed for a company needing to stay HIPAA compliant in a secure manner. Lot's of good features to compliment the form software (i.e. Scheduling, etc.)

Cons: It would be great if it had a point of sale aspect as well as a feature to hold electronic medical records (similar to practice fusion). If those two systems were able to be combined, it would be the perfect system for our business.

Overall: Intake Q has streamlined our New client paperwork to a simple, online form that can be completed and submitted back within minutes of a patient being seen. This saves us a ton of time and money (no need for paper forms anymore) and we're 100% satisfied with this. The fact that it's able to be placed on our website is another bonus. Highly recommend to anyone who is looking to move away from paper forms.

Pros: Scheduling, ability to connect with my google calendar, appointment reminders, ease of creating forms, ease of sending out forms, notifications when forms are completed and returned, the dashboard.

Cons: The patient documentation portal and customization don't meet my needs. It would be nice to be able to have my documentation connected to my schedule, though.

Overall: I use IntakeQ for scheduling, appointment reminders and new patient intake forms and consents. It has saved me so much time and helped to keep me better organized. It is so easy to use and very intuitive, and my patients all seem impressed by it. The customer service is also very receptive to suggestions and requests, which is reflected in the updates and new features. I don't use it for my documentation because it doesn't work for my needs, but that is an option for those who are looking for it.

Pros: IntakeQ has everything I need to run my clinic, skipping none of the features. I've spent the last month creating sheets to weigh in all of the options, with IntakeQ winning over and over again. The interface is clean, modern, and easy to use for both clients and clinicians. Adding assistants and non-clinical staff is simple, without any limitations to what they can see or interact with unless I create said limitations. The ability to use Zapier and export the API means those of us looking to really make something fancy are able to explore endless possibilities. For my clinic, which hinges on being state of the art, this is the perfect platform.

Cons: Clinical note templates and treatment plans leave a bit to be desired, especially in comparison to Simple Practice or Thernest- both of which have robust templates. This is really only a con if you find yourself regularly using quick labels or quick notes, which our non-clinical staff like to do. Though, our clinicians often correct said notes and customize them, which is something IntakeQ allows for.

Pros: I love IntakeQ! Before I started with IntakeQ, I did a deep search and tried different systems for my Massage/Coaching business.

My main criteria were to go completely paperless and I was looking for a fully automatic way: Client book the session/ or I manually book a client; then the system will send the Intake form (You can even create your own Intake forms, with your own questions and is so easy!), Reminders (both email and SMS to choose), online booking. And after the session, I can easily update my clients details from my Ipad or phone (SOAP notes).

The most importantly, the IntakeQ team is so responsive and always helpful, I asked for a few updates, and these were implemented in no time! Thank you so much IntakeQ team!

Cons: None! The IntakeQ system is perfect! And the team is implementing new improvements on an ongoing basis.

Overall: I can manage the whole client booking experience in one place. The latest addition to automatic birthday email is incredible! And just love Client Portal, place where clients can check their appointments, packages, invoices, etc..

Great customer service! Great functions but wish I could receive faxes as well as send them.

Pros: The functions and convenience~ I really like being able to send my intake packet on-line. My clients love this also because they are used doing things on-line. It is so helpful because I usually get the packet completed and returned the same day I send. It allows me time to prepare for a new client. I like NOT having paper and instead of having a comprehensive electronic client file. The appointment reminder for a first appointment is wonderful!

Cons: Not user friendly and not very intuitive. Great customer service but I would like to be able to do things without having to contact customer service! I would like to be able to click on a current form to see out something was created so I could then do it myself on other forms . I find my self looking at a form that was done for me my customer service and wondering "Now how did they do that?"

I really dislike that I can send but not receive faxes -- how great would this be to receive a client fax and be able easily to attach it to that file! I would even pay extra for that function,

Everything works efficiently and customer service is superb.

Pros: How easy it is for the patients to use. How amazing the customer service team is. No matter how small the issue they respond and fix the problem faster than any other customer service I have ever dealt with.

Overall: There are a lot of great features and has made scheduling so much better for our office. So glad to have gotten rid of the paper version of the schedule. We love the option for paperwork to be completed online as well as other forms. There have been a few glitches and some features we wish were available. We love seeing what updates come out each month.

Recommendations to other buyers: We'd like to be able to customize more settings, mainly that we can color-code appointments on the booking page. Basically, to indicate at a glance if everything is ready ex. could be green prior to the appointment and ex. yellow if records are needed to be received prior to exam or blue to still verify insurance. We also would like more space in the notes section on the bookings window. It would be nice if on the bookings window how there is a feature to "add notes" it's very helpful to also add a quick link to go to "overview" or profile. It would just be a lot faster than having to click add a note then cancel to see their overview/ profile or re-typing the name at the top to search clients.

Pros: I like that once it is set up I can just send it out and not worry about it. I also like that I can see it instantly once a client fills it out.

Cons: Difficult at moments to set up. Not everyone receives the email.

Overall: Overall, I am happy to have found IntakeQ. I have had a little difficulty with a few things while setting up my survey, because I am not a very tech-savvy person. But I was able to make it work. I compared prices of all of these types of services and this one was the best. I do a lot of online marketing and health surveys are a good way to filter out those with a dis-genuine desire for the work (I'm a female massage therapist). I would recommend this to friends and family, particularly those in the massage field.

Recommendations to other buyers: If you're not sure what to put in your intake (I wasn't), google various intakes and combine the best aspects of what you find. Explain to clients that your thorough nature is to ensure the best service and that it will only need to be done once. When you're done, send it to yourself and time yourself so that you can say it will take __(your time)__ to __(your time plus 5 minutes)__ to do. This will also allow you to see what they see. I have referred back to my intake more than once.

Overall: There are so many options out there. I am SOOO glad I found intakeQ. It makes my day go much more smoothly. My patients are able to complete and sign all my intake forms and consent forms. I can reject an incomplete intake and ask the patient to complete it. No paper is involved. I also use intakeQ to accept payment for my services.

Customer service is the best of any software I use. They graciously listen to my recommendations to improve my work flow and are very professional.

Overall: IntakeQ has been essential for our office in digitizing the intake process and allowing patients to book online, which has saved a ton of time previously spent on administrative tasks. Online forms have been instrumental in saving us and our clients time, effort, and the hassle of printing/scanning/faxing. The automated emails are very useful as well. If the service integrated with our EMR, our intake process would be almost entirely hands-off for administration. Support is great, very responsive, and new features are added very regularly. Definitely, would recommend the service to any practice looking for more automation in their intake process.

Overall: I shopped around a bit when starting my practice and ultimately went with IntakeQ because of the easily customizable intake forms and simple to use scheduling. As my practice has grown, IntakeQ continues to grow too. Every time I think "hey, it would be nice if my software could to this," I check the updates, and they've already thought of it. Cons: The software is expensive. I would like a plan between the Small Team and Clinic for 3 practitioners.

Overall: I use this software every day and it makes my practice run smoothly and I appreciate that it is Hipaa compliant. My clients like the opportunity to complete the forms in the comfort of their home and have it done when they come into my academy. The customer service is outstanding, especially Mike. Pros: Being able to set things up and send quickly to clients. I really like the online booking calendar, this has helped me a ton! Pros: Easy to use, and not tied to any reimbursement platform - this is great for a health professional who is not working with insurance claims. Those products linked to insurance reimbursement seem to be average in doing what IntakeQ does great!

Pros: The convenience of it has made going paperless fairly easy.

Cons: It does not work well with out Tablets and Ipads.

Overall: I run and manage a high volume chiropractic and wellness office. We see 350+ patient visits per week and in May of this year, we switched to a paperless office. Intake Q made the transition bearable. Customer support was very helpful in editing our forms and went above and beyond to assist us. Now if only Intake Q offered an app it would make using your services with an IPAD or Tablet so much easier. Also making it so we can send the signer a password would be helpful as well other than that I Love INTAKE Q and tell everyone who wants to switch to paperless to give them a try.

Pros: Once it's setup, there's very little maintenance. It's very easy to send forms out to patients and the information we can collect electronically really saves us time.

Pros: I love how easy it is to make my own forms and templates. I love the affordable price as well. Intake Q is the best Stand alone Software I could find after 4 years of research. It is simple practical and makes so much sense. I have zero programming experience but knew this was exactly what we needed after just a few days on the free trial mode! I signed up before the free trial was even over. Also I love how they have actually taken some of my suggestions and made them features! Bravo!

Cons: I wish there were a few additional features such as if a table equaled a score that the score could mean something. Also a better way for clients and provider to use the same form (kisok mode??) And allow for color customization as well for the side bar where they are just blue and green right now. Oh and a little more security on who can delete what. that would be good.

Pros: Excellent forms creations, including the ability to electronically send & receive to client. And with that, so many options too, such as sending a link by email or having it embedded right on your website. But it doesn't stop there, their amazing online booking program works flawless. Pros: From the moment we inquired about IntakeQ it was evident that the customer service was exceptional. They walked our team through the pros and cons of the platform and took time to make sure we understood how it worked, and what the specific application would be for our practice. What I appreciate most about support team is we asked them which plan would best suit our organization. It was refreshing to see someone who was 100% honest. They did not try to up-sell and said that the lower of the two plans would fit our organization perfectly.

Adding the forms was a breeze and it has been amazing for our office staff and the patients we see.

Cons: I wish that either they offered a full package to run a private practice, or made the forms and other items available at separate price points since I only use the forms.

Overall: I use this software for the paperwork in my private practice as a psychologist. A very easy form building and usage system. There are pre- built templates and questions that can be used and a very intuitive form building process. Terrific help from support converting some of my existing paperwork. Super helpful and reasonably priced. The workflow is a dream with client information and it is all HIPAA compliant. There are other scheduling and note feature included that I do not use since I have other software for that, but it's well worth it just for the form capabilities. You won't find anything else out there that is this good at this price point. Bravo!

Recommendations to other buyers: Shop and compare, but I doubt you'll find an HIPAA compliant form management system that's this inexpensive let alone this easy to use.

IntakeQ has excellent customer service; they respond to questions very quickly, they will help you get everything set up or they will provide step-by-step instructions if you're the DIY type. 2. Saves a lot of paper which helps your wallet and the environment. 3. Saves time and makes you more efficient; you don't have to scan and attach documents manually for example. 4. Helps you stay on schedule; most patients have the paperwork done before they come in for their first visit so you can have the chart ready to go when the patient checks in.

Cons: 1. There is quite a difference between the plans offered. The lowest one allowing for 10 submissions, then jumps to 150 submissions just one level up. This puts us "between plans" based on the volume at our clinic. 2. The cost to use the secure portal seems a little high.

Pros: Online forms, notification the form was received by the patient and they began filling it out. Don't need to have the latest update for Adobe, easy to fill out and read through the form.

Overall: I was using a fillable pdf for years. There would be multiple problems with people not having the updated version of Adobe, or they were computer competent enough to know how to fill out the pdf, save attach and email, or my pdf forms would get corrupted every few months and I would get multiple answers for the same questions, or the patient would fill out the form and save it and their information would disappear when they would re-open the file. Just a myriad of problems like that.

When I discovered IntakeQ, that resolved all of those problems. I love that I get a notification that they received the form and began to fill it out, I could re-send if necessary and gives me a notification when I receive it.

They also boast the BEST customer service I have ever experienced; any assistance needed with the software is attended to within 24 hours but usually MUCH sooner. Overall: I cannot rate this high enough. We used Docusign for years but there were always issues and it was very cumbersome doing most of the work yourself. We searched for an alternative online Signature site and there were so many. However, IntakeQ was the most superior of them all. At first I was worried about no phone support but let me tell you, that has NOT been an issue at all. As soon as you email anything, within mins - sometimes seconds - you get a reply. It is awesome and they are SOOOOOOO helpful and so eager and willing to assist with any questions. The best part is they do most of the work for you YAY. I can't say enough good things about them, they truly are a lifesaver. Plus it is so simple to use and they can customize things to suit your needs which makes them much more flexible than any of the other providers. Do yourselves a favor and look no further. Comments: Their Intake system is great! The pricing is reasonable. The Booking System is below average to average. If they can implement some of the features available on BookFresh - then they will have an awesome overall product. I don't find their booking system user-friendly: it doesn't have an area for you to leave notes for your customer when you are confirming their appointment. It doesn't allow you to easily search for client appointments. It doesn't show you a summary of actions/history of appointments. I think if there were to try a trial of BookFresh and bring their booking system up to that level, they will offer a better Booking System. Book Fresh isn't perfect, as it has a glitch with sending emails to you, the vendor, when a client books multiple separate appointments in one sitting. The emails to you as a vendor get lost. However, I still think they are better than the system offered by IntakeQ. IntakeQ has great customer service and they respond to you immediately.

Perhaps they should consider removing Booking system, if they aren't going to offer the best.

Pros: The fact that it allows me to be paperless which significantly streamlines my practice and cuts down on my need for paper storage.

Cons: I wish I could have my patient's families have unlimited access to certain forms (release of information) because they often need to complete this for multiple providers. This requires me to send them multiple links for this file.

Overall: I have been using IntakeQ for several months now, and I am very happy with the ease of use of this service. I made a goal to go paperless when I entered private practice and having all of my forms in electronic format has helped me to achieve this goal. I have received positive feedback from the families with whom I work whose feedback is ultimately the most important to me. I do wish that families could have unlimited access to some of my forms (e.g., release of information) because I often needed them to complete this form 2-3 times, which requires me to resend the link.

Cons: I would love to add membership contracts to this software.

Overall: I love how easy it is to send off intakes to clients! Previously we would send a word document through email to be completed by our members and they would have to print it off or deal with reformatting the document after completion. With IntakeQ our members are able to complete the forms online or on their phones with ease. They can submit them before their appointment, giving our coaches time to review their histories before hand and give the client more one-on-one time in their session.

Comments: I have been using IntakeQ for about 6 months. In general, it gets the job done. I have a few needs within my psychotherapy practice that it doesn't meet: 1) I can not create a document with two separate signature fields (i.e. when 2 partners in a couple want to sign a release of information). This is a recurring need so I find myself frustrated with having to create two documents for them. 2) you can't partially fill out a form for a client before sending it to them. This often requires I send a separate email explaining to clients how to fill out 'my part'. 3) Customer service is all by email. This can make for a lengthy trouble shooting process when you really need them. I found out they will schedule a time to help you if you schedule in advance however my 'issues' have popped up last minute and it's when I don't have a phone number to call. The employees are VERY nice and helpful when they do communicate via email.

At first, I didn't really see how it could possibly have forms that just like what I'm used to...until I sent over my form for them to transfer over. It was done so quickly! And it looked great! I had a beta group of patients to try it out first and they complimented immediately on the ease of use. Whenever I come up with an idea or think I need a different feature, they either already had it (and I didn't know it yet!) or they provided another solution until it could get added. Pros: I love the ability to modify and create the intake forms, the online booking is easy, and the automated emails are great.

Cons: Customers have difficulty when they don't complete a form going back to it, not sure why. I'd like to make charges not attached to an appointment but that's not that big of a deal. The thing I would change and it's super tiny, the manage appointment link is not very visible and people don't realize they can cancel/reschedule etc.

Comments: IntakeQ has revolutionized our intake process. It has dramatically reduced the amount of time our office staff spends on the phone with potential new clients because we are now able to instruct our clients to provide all the necessary information via the secure, HIPAA compliant IntakeQ form. We also have been able to streamline our assessment process and shave a full 30 minutes off the appointment time due to having the consent forms and background information forms already completed before the appointment. The additional feature of being able to download completed forms from IntakeQ in PDF format allows us to easily upload the forms into our practice management system.

Overall: Using intakeQ has helped my office to move more quickly and efficiently. It allows me to task out my office staff for other projects, saving us time and money.

Recommendations to other buyers: It is very easy to customize so be sure to check in with your clients and staff to see if you need to make any adjustments as you implement the software.

Comments: I heard about IntakeQ from another holistic practitioner. I tried to create my own questionnaires but it was far easier to pay extra for IntakeQ to create and customize them for me, with my own logo. Sometimes my clients complain of errors and difficulty submitting their responses, which is frustrating for them and me, especially when I find them complete in the "Not Completed" file. Even when there have been problems, the customer service is prompt and friendly. I have not integrated the schedule into my website, so I cannot comment on that.

Recommendations to other buyers: You should be aware that there will be an additional foreign transaction fee if you are a US-based user. Overall, it's satisfactory but could be improved.

Comments: I researched options for automatic reminders for patient appointments for awhile, but many solution require you to use broader, more expensive EHR software or are not Mac compatible. Intake Q is entirely web-based so it is cross-platform, and although I would like to see more features the team has been working on feature development actively over the months. It has been relatively easy to integrate with my workflow, and I also use the system to enable clients to fill out intake paperwork online which has some real advantages. The cost is still significant for my practice but the convenience advantages make it worth it to me, and my clients tell me they appreciate getting text reminders.

The BEST solution for patient forms, integrated IM, and even custom EHR!

Pros: That it offers everything under one brand, low cost and BAA. I have been patch working a number of applications and software programs to get only a fraction of what I am getting from IntakeQ. I could not be more excited about finding them!

Cons: I think that the software could have a better layout on mobile devices or an app to use.

Pros: Ease of use, mobile first design

Cons: No way to embed forms into the website - available through link only. We would love a feature to embed the full form onto a web page or secure patient portal without going through an IntakeQ page.

Overall: We love IntakeQ! Although we don't use all the features, the forms component is solid. Our company has been using the service for months with no problems.

Pros: So easy to use and set up our forms. Super customizable. Our patients love that they don't have to print out paperwork or come early to their appointments. I would highly recommend!

Cons: mac users seem to have a hard time submitting forms. Patients often complain of getting stuck and being unable to submit completed forms.

Amazing! Love having our forms online! Saves time, money and sanity!

Pros: Easy and not expensive at all.

Cons: has to have an email to send the paperwork

Overall: Before online forms for our office we spent hours on end scanning, uploading, and shredding office intake forms. NOW we don't waste paper and forms are filled out more in depth than the paper kind. Highly recommend for the money you save and spend on the forms.
