Amazing. Whenever we have an issue, they are quick to respond and help out. Product is very easy to use and understand.

Pros

It allows us to streamline many aspects of our workflow and is highly customizable. It is great for forms sent to our patients and for internal forms that we use amongst ourselves.

Cons

Cannot control the reminders that get sent to patient's easily. It would be great to be able to specify whether reminders get sent with particular forms.

It has been very good to our business. Despite the few things that need done better (here and there), the overall system has made things much easier for us all at a growing practice.

Pros

It has become very easy to use. We do not need to reach out to our rep as much since we can update and change the forms easily as needed.

Cons

It seems to take a while for the ball to get rolling on features that are coming out to users. I am unsure if it is on a massive list and there is only so much that can come out at a time but calculating scales took about 2 years from when we heard it would be done.

I recommend Zentake to anyone looking for Form Automation. It's very easy to create/customize forms, for the patients to complete the forms and for us to receive the forms back. Great customer service. It's been a Great Experience overall!

Pros

What I like about the product is: - It's very easy to create forms from scratch or customize the forms after being created. - User-friendly. You don't need extensive training to start using the program. - Ability to send individual forms or organize them on a packet. - Ability to have Required fields on the form. This will help to collect all the necessary information. - It's easy for patients to fill out the forms. No complaints from patients so far. - Customer Support - \[sensitive content hidden\] is our contact/support person. He always goes above and beyond to resolve any issues or answer questions we might have. - It will take less than a minute to send a form/packet to a patient. - HIPAA compliant In Summary: It's a great product for form automation, easy to use and customize. Customer service support has been great. Very responsive.

Cons

I have no Cons at this moment. So far so good. Any issues were fixed right away.

Pros

Zentake far exceeds all other integrated form platforms I have used. Its ease of use and customizable fields coupled with its price makes it hands down a go to product.

Cons

If I have to choose something to dislike about Zentake it would be that there is a learning curve to create forms. There are so many options and ways to manipulate how to present your forms that it takes time to learn the system and all it is capable of.

A game changer for my micro practice. The price is incredible for the product and the service I receive has been incredible.

Pros

I have a functional medicine practice which require detailed information from patients. I was struggling with making my micro practice easy and functional for the patient to complete all the laboriously required data either in advance of their consult or follow up appointments. Zentake was suggested to me and believe me, it has absolutely changed my practice in a positive direction. Fillable forms are easy for the patient. No limit to the characters. Dashboard is easy to use and interface with. My rep, \[SENSITIVE CONTENT\] couldn't be more responsive to my needs from designing any and all my forms to helping me design my own, now that we have the capability.

Cons

The only way I see this software being better is if it could integrate with my EMR. But outside of that, I wouldn't change a thing.

That you can create custom forms, the data can migrate and populate to the discrete fields in the EMR. I love the customer support

Cons

The signature feature on an iPad is touch and hard for my elderly to do

Great! Easy onboarding. They created a lot of the forms for me which took a load off. Steven is verry responsive and available!!

Pros

The ability to create developmental forms so parents can complete them at home. Also the ability to integrate that information directly into the EHR which means less chart prepping for my staff

Cons

The input of of registration forms from Elation EHR is time consuming to sign off on . I wish there was a way to assign forms that did not need to be signed off

Pros

The software is very easy to use and our patients have no complaints about it either. It allowed us to customize the frequency with which we contacted them.

Nothing. I tried to find something about it that I didn't like but came up empty handed each time. We have used it going on two years.

As a primary care clinic, we use alot of forms (patient intake, questionnaires, health histories, screening tools, etc) that were formally on paper. We asked Zentake to make them all electronic so that patients can complete on an iPad. It has been great so far.

Pros

Everything - professional / personalized quality of the forms, excellent and prompt customer service, ease of use.

Cons

That many of the forms have to be saved and then copied and pasted into our electronic medical record - we are hopeful that we will eventually be able to skip this step.

Overall, Zentake has lived up to what we were told we would get. Help is easy and quick, and the packets save us so much time.

Pros

Zentake is easy to use for our office staff and clients. When we have problems, they are also resolved quickly.

Cons

Sometimes it won't let us download a packet as one file, and that is a pain to do each form individually.

Let me tell you, this has been a great experience. I noted two needs that were needed and had a quick resolution with it the week. Well done. This is probably all for now, but I'll certainly think of other things that would be helpful for me and other providers or clinics or other businesses out there that use your platform

How it seamlessly adds the appropriate info to the location in the chart or note prep. Fillable PDF is great too.

Cons

Nothing yet! If anything I’d have a per clinic price and Not per provider . . . .

I have struggled for so long trying to find the perfect fit for intake forms and progress/SOAP notes that it isn’t even funny. No intake form had the right stuff on it, and other intake software didn’t take into account that my state requires a therapist signature as well. Or they were super clunky/ugly/ not client end friendly. (Just because it’s healthcare, doesn’t mean it has to be old and outdated looking). I also struggled with standard SOAP notes. Not anymore! A few of my clients (yes, I had clients who have zero problems telling me what they don’t like, test these) now joke that I have the best forms and form filling set up around 😅. I was even able to finally have the SOAP / Progress note set up that I have been searching for. Overall experience has been super positive, and they have for sure exceeded any expectations. Much appreciated. Adding here that I started with Square Contracts or paper/pen, and other software tested: PracticeQ/IntakeQ: both similar in pricing, but I couldn’t get past the client needing a password to fill a form (friction area for the client), as well as JotForm: therapist signature was friction for me, and the price increase as soon as HIPAA is mentioned were all a no thank you for my particular needs.

Pros

I really like the amount of options for customizing your forms. They have adaptable / conditional response options, showed me how to break up a form so it doesn’t seem so obnoxious, clients can pause and continue later.. etc. Clients can fill, then it goes to your main dashboard for any office use areas / practitioner signatures, etc. to be filled. Also, their customer service is amazing (and fast). I didn’t follow initial directions on having forms automatically import to a drive (after reading the easy to understand guide, it was super clear to me that I was the error, lol.) They were super cool about it and answered all the side questions I had. I needed a certain set up / importing order for a form, and they literally created a beta feature for me to test. That was amazing (and it works perfectly). I also really love that they don’t force password protection just to fill out a form as a client, since that would create a friction point for the client/ patient. The client side is super user friendly, aesthetically pleasing, easy flowing, and they have options for them as well as far as allowing downloads of the forms they filled, etc. (which you, the form creator can allow or not). They have quite a few integrations available as well. So glad I decided to test them. I now have the most functional (for me & easy for clients) forms in my area 😂. Internal forms, office use section, packets (multiple forms that can be sent), signatures, and all the things. They are all optimized for whatever device (phone, tablet, desktop) the person is using. Tablet / kiosk modes (I use those for in office form filling). They’re great and I do not regret choosing them at all. Cost wise.. I feel it’s mid to slightly lower than other similar software costs. (For all that you get, and the amazing team behind Zentake, I feel it’s worth the cost).

Cons

I haven’t found anything that I dislike, and if I did, I would simply contact them and ask, since their team is amazing.

Pros

Zentake interfaces without electronic medical record thus has significantly reduced our scanning

Cons

we have multiple clinic locations. Currently Email notifications of completed documents all go to 1 person who forward them to the appropriate clinic. Since the documents or packets identify clinic locations it would be great if the Zentake system sent each locations email notifications to a different email address but it's a minor inconvenience since the documents upload to our electronic medical record and it's just a notification.

Pros

Zentake's integration with Elation is fantastic, and the conditional logic feature really stands out. It allowed us to significantly enhance and personalize the customer experience, which was a big win for us. What really set Zentake apart, though, was the onboarding process. Often, when you sign up for new software, you're left to fend for yourself. That wasn't our experience with Zentake at all. The support we received was exceptional, far exceeding my expectations. This level of support is a game-changer and a primary reason I'd recommend Zentake. And when you add in all the other features Zentake offers, my high recommendation comes as no surprise.

All of my concerns were addressed. This is a highly responsive company dedicated to great customer experience.

I have had a really positive experience with Zentake. The software is really easy for us to use and it's more intuitive for the patients than having to download individual forms to their computer and then send them back to us. I can't say enough about the customer service I've received from \[SENSITIVE CONTENT\] . He's very responsive, positive, and I'm so glad he's on our team.

Pros

It has made it so much easier for patients to fill out required forms since we are doing more and more telemedicine appts. Before this, patients were very confused and got frustrated easily.

Cons

The only issue we've had so far is some patients' computers were auto-filling info into the name boxes on the forms. But I was able to talk to Stephen and he got it fixed right away. We have not had any other issues since then.
