5thPort is a digital patient education, engagement and eConsent platform. It can be used across individual practices, hospitals, health systems, and in clinical research. The platform utilizes high quality, prescriptive multimedia and teach-back to educate patients and their caregivers on their diagnosis, care plans, risks and benefits of recommended procedures. Once the patient is well-informed, our digital eConsent process helps to facilitate a meaningful conversation with your healthcare provider. The analytics collected by our platform are a significant source of understanding of each patient's level of engagement and their understanding of their disease, treatment options and associated risk/benefits, allowing providers to focus on patients who had difficulty with comprehension. 5thPort is multilingual capable, so engagement can be done in the patient’s language of choice, reducing socio-economic and cultural barriers.

5thPort Reviews
"5th Port software / technology has revolutionized my surgical practice."
Overall: I'm absolutely pleased with the 5th Port platform. As a busy surgeon who generally sees 40+ patients a day, 5th Port helps me deliver a standardized consultation in an efficient process that is highly documented. For the first time I feel confident that every patient is fully informed about their treatment plan and associated risks and benefits before I begin the procedure.
Pros: The software allows me to deliver standardized, high level consultation content that is time and date stamped for documentation. First technology I have found that documents the content the patient views. On the backside of the consultation 5th Port provides procedure specific teach -back to document comprehension. Lastly, the platform dynamically generates a digital informed consent based on diagnosis and procedure. The first and only software to dynamically generate these paperless forms. My staff can generate 40 digital consent forms in just 35 minutes.
Cons: I can't think of a drawback to this technology.
"5th Port has been efficient and easy to use"
Overall: Since implementing the 5thPort into our practice work flow, the shorter timed videos gives nursing staff and doctors more time to dedicate to clinical care.
Pros: The software has been easy to use, saves time and paper.
Cons: The elderly patients have a hard time with the upgrade.
"Great Education Tool"
Overall: 5thPort's patient education process has been given high satisfaction ratings and we often hear how much patients appreciate the detailed information presented in the videos and the simplicity of the software.
Pros: The teach back feature is a great tool for patients.
Cons: The biggest challenge that we have faced is using his software on tablets. The older generation tends to get frustrated with the device itself.
