"All in One Place"
Overall: Overall I am pleased with all we can do with this software in one place.

Overall: If all the kinks are worked out, I can see Formstack Platform being a valuable tool for any business. However, in my experience, it did not live up to the expectations.
Pros: Formstack provided us the ability to register hundreds of patients' medical forms with ease, thus eliminating the need for physical paperwork. Patients could fill out their forms on iPads, and we in turn could readily see all our patients for the day.
Cons: Too often the app would have issues that we would then have to seek management/IT to resolve. Whether it be missing patient information, inability to upload documents, or just the app crashing itself. In my brief time using the platform, it certainly was underwhelming.

Overall: Overall the suite worked and is a good solution for what it was designed for. It almost met all of our needs. There were problems with the app not working correctly offline (you had to be online to open the app), and with signing on a touch screen computer. Finding bugs in a system is not a big deal, that is to be expected. It is how a company responds to the problem that matters. A buggy system with good tech support who actively try to fix issues when notified, still gets 5 stars, but a system where 6 years after a plugin has been patched they still have not implemented the fix does not. No Matter how good the rest of the system is.
Pros: It provides a nice interface to handle collecting information and signatures. The learning curve is a little steep but once you get the hang on it, the process is not too bad. The system is stable, we never had an issue with crashing or loosing data. There are several ways to collect data, complete documents and collect signatures including embedded forms, web links, a mobile application, etc.
Cons: I did not like that you had to switch between different applications in order to complete a document. We had to use FormStack Forms to collect the user information and signatures in a web form, then this data including the signature was transferred to Formstack documents in order to generate the completed and formatted PDF. This results in the document that the client signed is not really the final document. They signed a web form and then their signature was transferred to a different document prior to them ever seeing the other document. I also did not like the lack of desire to fix bugs even with handed the solution on a silver platter. In March 2021 I identified a problem where when using a windows touch screen computer to sign on a webform, the form would interpret the movement as scrolling instead of signing and just move the page up and down. I went as far as to identify the specific cause of the bug as an outdated version of the jSignature plugin and provided them the solution. The author of the plugin had fixed this issue back in 2018 (github.com/brinley/jSignature/issues/157) but FormStack still has not implemented the fix. After over a year of not being able to fully use the system because we could not collect signatures on a windows touch screen computer we switched to a different platform.
